Sir, I am hepatitis b liver cirhossis with portal hypertension and small esophageal varices dilated portal vein and splenomegaly I am using cardivas 6.25 mg twice a day I used only bp tablets before I diagnosed this now doctor told me to stop using bp tablets and experiencing low bp 90 and 40 what I need to do now.

Hi, cardivas is needed for you to prevent bleeding from varices. You can get low bp due to cardivas. You need to see your doctor and get the dose adjusted.

Sir I am suffering from hepatitis b which diagnosed 1 year earlier then after scanning it shown hepatitis b liver cirrhosis with portal hypertension, splenomegaly and gallbladder stone. I have gone endoscopy and doctor told small esophageal varices 40 percent liver damaged, ulcers. After treatment gall bladder stones, ulcers gone. But splenomegaly size increased now in ct scan it shown hepatitis b liver cirhossis with features of portal hypertension dilated portal vein. Doctor changed cardivas 3.125 to cardivas 6.25.now what I need to do. What is my life expectancy, can I go liver transplant now does it can make me live normal life or can I go with medications my age 50. Please suggest me. Sir what is life expectancy.

Since you are in ahmedabad. Few suggestions. 1) continue treatment of hep b 2) fibroscan can help you get the assessment of liver stiffness in case you don't have ascites on usg. 3) liver transplant is an option when liver decompensates but you need to enroll early on as waiting period is long in most centres 4) get consultation from Dr. sanjay rajput. He is expert gastro in ahmedabad.

Hi, from the details provided by you, I understand that you have cirrhosis of liver with chronic hepatitis b infection as the cause. You do not seem to have developed complications of portal hypertension viz. Gi bleed (blood in vomitus or stools), ascites (collection of fluid/water in stomach) or hepatic encephalopathy (drowsiness, transient loss of consciousness) etc. You need to continue with anti-viral medications for hepatitis b and cardivas along with regular screening and follow up with blood tests (every 3 to 6 months), ultrasound of abdomen (every 6 months) and surveillance endoscopies. Liver transplant is usually needed once you start developing complications or liver cancer (hcc) and is decided by your gastroenterologist/ hepatologist based on several parameters and scores (meld score.)

Sir, if the patient gets the hbsag test done on 61 days, then this test will be considered conclusive or not. According to cdc. Window period of hbsag is average 4 week (1-9 week) after exposure.
