About Losar-H Tablet

This is a combination medication which has Losartan and Hydrochlorothiazide as active ingredients. It belongs to the angiotensin II receptor antagonists group of drugs. This prevents the blood vessels in your body from narrowing, thus lowering your blood pressure and improving the blood circulation. It can lower the possible risk of a stroke in people suffering from any heart condition. Apart from that, it can also slow down kidney damage among people with type 2 diabetes.

There are three main reasons for its usage. Firstly, it is used for the treatment of high blood pressure. The second usage is for reducing the risk of stroke in people, who already have high blood pressure as well as left ventricular hypertrophy (LVH). Lastly, this medication is used for treating diabetic nephropathy, a kidney disease caused due to diabetes.

There are some common side effects of this medication that are generally mild and go away in a few days. They include:

  • Diarrhea
  • Fatigue and dizziness
  • Pain in the chest
  • High/low blood pressure

    How does this medication work?

    This medication displaces angiotensin II with very high affinity from its binding site at the AT1 receptor subtype, which is responsible for the known actions of angiotensin II. Hydrochlorothiazide is a diuretic which inhibits a sodium-chloride symporter SLC12A3 in the distal convoluted tubule to inhibit water reabsorption in the nephron. The symporter reabsorbs sodium and creates an osmotic gradient for water to be reabsorbed, inhibition of which prevents water reabsorption.

        Alcohol

        Consumption of alcohol with this medication will lower the blood pressure and can cause dizziness, headache, changes in heart rate. Avoid driving or operating heavy machinery if you have these symptoms.

        Use of these medicines is not recommended especially in the elderly population with diabetes and kidney disease with CrCl less than 60 ml/min. An alternate medicine should be considered under the doctor's supervision.

        Captopril

        Use of these medicines is not recommended as it may increase the risk of kidney impairment and low blood pressure. You may experience weakness, confusion, and irregular heartbeat if these medicines are taken together. Regular monitoring of blood pressure and kidney function tests are to be performed. An alternate medicine should be considered under the doctor's supervision.

        Dexamethasone

        The desired effect of this medication will not obtain if these medicines are taken together. This interaction is more likely to happen if dexamethasone is taken for more than 1 week. Inform the doctor if you have a sudden weight gain, swelling of hands and feet. If coadministration is needed then appropriate dose adjustments are to be made under the doctor's supervision. Regular monitoring of blood pressure and kidney function tests are to be performed.

        Diclofenac

        The desired effect of this medication will not obtain if these medicines are taken together. The risk of kidney impairment will increase if these medicines are taken together especially in the elderly population or having pre-existing kidney disease. Inform the doctor if you have an increased or decreased urination and unexplained weight gain or weight loss. Regular monitoring of blood pressure and kidney function tests are to be performed.

        Insulin

        The effect of insulin will increase if these medicines are taken together. You may experience dizziness, headache, sweating if these medicines are taken together. Regular monitoring of blood glucose levels is needed. Suitable dose adjustments or an alternate medicine should be considered under the doctor's supervision.

        Congestive Heart Failure (Chf)

        This medication is used with caution in patients with congestive heart failure especially in volume or sodium depletion. Transient hypotension is also not a contraindication to further treatment with AR antagonists since therapy can usually be reinstated without difficulty after blood pressure stabilizes.
