एस्टिग्मेटिज्म क्या है?

एस्टिग्मेटिज्म का मतलब है कि आंखों की पूतली पूरी तरह गोल नहीं है। हम में से अधिकांश को कुछ हद तक अस्थिरता है। लाइट एक सामान्य आंख की गेंद में प्रवेश करता है और एक स्पष्ट दृश्य देता है, समान रूप से झुकता है। हालांकि, अगर आपकी आंख का आकार घुमाया जाता है, तो प्रकाश एक दिशा की तरफ झुकता है। इसके परिणामस्वरूप फोकस में कुछ भी हो सकता है। दूरस्थ वस्तुएं धुंधली दिखाई दे सकती हैं। एक नेत्र रोग विशेषज्ञ के लिए शल्य चिकित्सा, चश्मा या संपर्कों का उपयोग करके अस्थिरता को ठीक करना काफी आसान है। लोगों के पास यह असामान्य नहीं है और ज्यादातर मामलों में लोगों के जन्म के बाद से यह होता है।सटीक कारण ज्ञात नहीं है। सर्जरी, आंखों की बीमारियों और आंखों की चोट भी इसका कारण बन सकती है। टीवी के बहुत करीब बैठकर या खराब रोशनी की स्थिति में पढ़ने से इसका कारण नहीं बनता है। यदि आपके पास धुंधली दृष्टि है तो उचित आंख परीक्षा की आवश्यकता हो सकती है क्योंकि इसका मतलब यह भी हो सकता है कि आप दूरदर्शी या नज़दीक हैं। चूंकि अस्थिरता के लक्षण धीरे-धीरे प्रकट होते हैं, दृष्टि स्पष्ट होने पर आंख डॉक्टर की एक यात्रा आवश्यक है। संपर्क या चश्मा अस्थिरता के अधिकांश मामलों को सही कर सकते हैं। यदि अस्थिरता की सीमा बहुत कम है, तो सुधारात्मक उपायों की आवश्यकता नहीं हो सकती है। कॉर्निया आंख का अग्रभाग हिस्सा है और अनियमित अस्थिरता में प्रभावित हो जाता है, जो काफी असामान्य है। केराटोकोनस एक ऐसा प्रकार है जहां आमतौर पर गोल कॉर्निया शंकु के आकार को बदल देता है।

आम प्रकार के अस्थिरता के लिए दो उपचार उपलब्ध हैं:

सुधारात्मक लेंस- ये कांटेक्ट या चश्मे हैं। यदि अस्थिरता आपको प्रभावित करती है, तो डॉक्टर एक प्रकार का मुलायम, विशेष कांटेक्ट लेंस (जिसे टॉरिक भी कहा जाता है) निर्धारित करेगा। वे एक दिशा में एक बड़ी डिग्री के लिए प्रकाश मोड़ सकते हैं। यदि अस्थिरता की डिग्री गंभीर है, तो एक कठोर गैस-पारगम्य लेंस की आवश्यकता हो सकती है। नेत्र रोग विशेषज्ञ आपके लिए सही चुनने में सक्षम होगा।

अपवर्तक सर्जरी - इस लेजर सर्जरी के कारण कॉर्निया का आकार बदल जाता है। कई प्रकार हैं, इसलिए एक नेत्र रोग विशेषज्ञ आपको सही चुनने में मदद कर सकता है। ऐसा करने के लिए कोई कॉर्नियल निशान या रेटिना समस्या नहीं होनी चाहिए।

I am a 41 year old woman. I have suffered from generalized anxiety for years, treated with desvenlafaxine 50 mg/day and clonazepam 1 mg/day, very well managed and without panic attacks for years. A year ago, shortly after having cavity, I started having a lot of fatigue. At the end of december 2022, the fatigue was disabling and I began to have paresthesia, a lot of pain, brain fog, and hyperacusis. I had many tests done and finally in february I was diagnosed with post viral chronic fatigue syndrome due to epstein barr or covid and was treated with modafinil 400 mg/day, magnesium 400 mg/day and vitamin d3. Two weeks ago I saw a new neurologist and told him that I was in a lot of pain. He put me on lyrica-pregabalin (i'm on 50 mg/day now but need to increase to 75 mg tomorrow) and 20 mg transdemic buprenorphine patches for a month, a treatment he often prescribes for fibromyalgia pain. He told me to take this new medication without removing any drugs that I was already taking. Everything was going well until 5 days ago I started feeling weird. I have been sleeping very badly for 2 days and I am very worried. I have a hard time falling asleep and in the end I sleep like 4 hours a day waking up a thousand times during the night. Today I looked at the graphs of the apple watch during sleep and I have dropped from an average of 13. 7rpm (breaths per minute) two weeks ago to 9.8rpm now. Tonight I hit 8rpm and 91% saturation. I have never had such low data until I started this medication. Now I read that transdermal buprenorphine can cause serious breathing problems especially if taken with benzodiazepines and lyrica. The doctor says that he sees no problem in the data but if the patches don't help my pain, I can stop them. They help me a lot with the pain, but I am afraid of the side effects. In addition to the issue of sleep, it gave me great difficulty urinating and swallowing. What do you think? Should I suspend the patches?
