Addiction - The Ayurvedic View
Ayurveda considers alcohol and nicotine consumed in large quantities as ‘Visha’ or toxic. According to Ayurveda, alcohol has the following properties-

  1. Light or Laghu
  2. Hot or Ushna
  3. Sharp or Tikshna
  4. Drying or Visada
  5. Rough or Ruksha
  6. Expensive or Vikaasa
  7. Swift or Aasuga
  8. Sour or Amla
  9. Subtle or Sookshma

These properties are all opposite to the qualities that ojas possesses. Ojas has all life-giving properties and alcohol and nicotine have life-negating qualities.

Why do humans get addicted to alcohol and nicotine?
Human brain produces Dopamine normally and what alcohol does is to activate the nervous system to promote dopamine formation. The brain thus remembers the taste of alcohol and nicotine by creating special cells called receptor cells. As a person increases the quantity of alcohol, these cells also increase in numbers. When the person wants to give up his addictions these receptor cells agitate and create withdrawal symptoms with unpleasant and even dangerous symptoms.

De-addiction - Ayurveda Way
De-addiction treatments in Ayurveda comprise the following:

  1. Purification therapies or Panchkarma: These are used for getting rid of all toxins present in the patient’s body. These must always be done under the supervision of an ayurvedacharya as they are quite potent. After Panchakarma, the patient is given vomiting therapy or Vamana, followed by purgative therapy or Virechana, Basti or enema therapy and administration of Nasya or nasal drops therapy. All these help in detoxifying the body and mind and prepare the patient for further de-addiction ayurvedic treatments like Rasayana therapies which nourish and strengthen the body after Panchkarma. For treatment of addictions, Rasayana treatments include changes in diet and use of oral herbal medicines both.
  2. Herbal medicines used in de-addiction: A range of herbal medicines and their combinations are used in the treatment of addictive problems in Ayurveda and these are to be consumed orally. Ayurveda prescribes Chamomile and Ashwagandha in combination to heal the lungs. These help in cleansing lungs of smoke- related problems. Doctors also prescribe congestion relieving herbs like ginger and pepper mixed with honey or lemon. 

In addition, herbs to improve memory, concentration and to fight depression are also given. These are:

  1. Shankhpushpi
  2. Kanphool or dandelion
  3. Brahmi
  4. Ashwagandha

In addition, herbs to treat sleep disturbances like Jatamansi, Sarpgandha, Brahmi and Jaiphal are also prescribed. Since most addicts have impaired liver function, herbs that improve liver function like Aloe Vera, Punarnava, Long pepper, Kutki, Tulsi are given to revive the liver.

Liver diseases such as cirrhosis, Hepatitis, Autoimmune hepatitis, PSC, Fatty liver, Alcoholic liver diseases, are not just dreadful, but they also cause irreversible damage to the liver. Modern medicine rely on transplanting the liver when its health reaches a point where treatment can no longer recover or stop its deterioration. So, a liver transplant is the way to go for modern medicine when there is end-stage liver disease. However, transplant is not a cure; rather it is an escape from the cure. Functions of Liver - The liver regulates most chemical levels in the blood and excretes a product called bile. This helps carry away waste products from the liver. All the blood leaving the stomach and intestines passes through the liver. The liver processes this blood and breaks down balances, and creates the nutrients and also metabolizes drugs into forms that are easier to use for the rest of the body or that are nontoxic.

More than 400 to 500 vital functions have been identified with the liver.

  1. Production of bile, which helps carry away waste and break down fats in the small intestine during digestion.
  2. Production of certain proteins for blood plasma
  3. Conversion of excess glucose into glycogen for storage (glycogen can later be converted back to glucose for energy) and to balance and make glucose as needed
  4. Regulation of blood levels of amino acids, which form the building blocks of proteins
  5. Processing of hemoglobin for use of its iron content (the liver stores iron)
  6. Conversion of poisonous ammonia to urea (urea is an end product of protein metabolism and is excreted in the urine)
  7. Clearing the blood of drugs and other poisonous substances
  8. Regulating blood clotting
  9. Resisting infections by making immune factors and removing bacteria from the bloodstream
  10. Clearance of bilirubin, also from red blood cells. If there is an accumulation of bilirubin, the skin and eyes turn yellow.

When the liver has broken down harmful substances, its by-products are excreted into the bile or blood. Bile by-products enter the intestine and leave the body in the form of feces. Blood by-products are filtered out by the kidneys, and leave the body in the form of urine.

The beginning of liver disease -

According to Ayurvedic principles mostly all diseases related to the abdomen are produced due to MANDAGNI (low/poor appetite) including liver diseases. Three main doshas VATA, PITTA and KAPHA are increased aggravate and obstruct the channels of sweat and other body fluids than accumulate in the digestive system disturb the whole abdomen and produce liver diseases.

  • Children: Mothers exposed to smoke (active or passive) are more likely to have children with motor control problems, perception impairments, and attention disabilities. Exposure to smoke early on affect the lungs of infant as early as the first 2 to 10 weeks of life, it increases the risk for lower respiratory infections (asthma, bronchitis, and pneumonia) by 50% and worsens the condition of children who have existing asthma.
  • Spine & Bones: Smokers whose jobs involve lifting heavy objects are more likely to develop low back pain than non-smokers. They are at increased risk for developing osteoporosis and rheumatoid arthritis. Smokers are more prone to develop degenerative disorders and injuries in the spine, they have more trouble recovering from spinal surgery as smoking also impairs formation of new bone.

Leave for 30 minutes and then wash off with lukewarm water. This will help in keeping your skin healthy and give you a lighter tone in natural way.

These skin lightening packs are good for keeping your skin tone lighter. You can also use some tips that are good for bleaching your skin. This can make your skin look whiter naturally. You must use the proper sunscreen after using these bleaching agents. Avoid using these agents around the eye area. Never use the agents in higher concentration other than that is recommended. Misusing these will harm the skin tone for a longer time period. Some natural bleaching agents are:

Lemon juice and cucumber juice mixed with a small quantity of turmeric powder.

Fresh milk of coconut and juice of pineapple mixed in equal quantity.

  11. Basal cell carcinoma: The most common type of skin cancer. Basal cell carcinoma is less dangerous than melanoma because it grows and spreads more slowly.
  12. Squamous cell carcinoma: A common form of skin cancer, squamous cell carcinoma may begin as an ulcer that won’t heal, or an abnormal growth. It usually develops in sun-exposed areas.
  13. Herpes: The herpes viruses HSV-1 and HSV-2 can cause periodic blisters or skin irritation around the lips or the genitals.
  14. Hives: Raised, red, itchy patches on the skin that arise suddenly. Hives usually result from an allergic reaction.
  15. Tinea vesicolor: A benign fungal skin infection creates pale areas of low pigmentation on the skin.
  16. Shingles (herpes zoster): Caused by the chickenpox virus, shingles is a painful rash on one side of the body.
  17. Scabies: Tiny mites that burrow into the skin cause scabies. An intensely itchy rash in the webs of fingers, wrists, elbows, and buttocks is typical of scabies.
  18. Ringworms: A fungal skin infection (also called tinea). The characteristic rings it creates are not due to worms.
  19. Skin Allergies – Reaction of body through the skin to some allergens like some food item, dust, sun etc

A Guide to Living Healthy: Preventing Cirrhosis of the Liver!

Cirrhosis of liver is slow and gradual replacement of normal healthy liver tissue with scar tissue which results in poor liver function and blockage of flow of blood through liver which comes from intestines. As more scar tissue replaces normal healthy liver, liver begins to fail.

What causes cirrhosis?

  1. Alcohol: Amount of alcohol which can cause liver damage varies from person to person, however those who consume alcohol for long in significant amount are more prone to develop liver damage.
  2. Hepatitis B or hepatitis C: Caused by hepatitis B and C viruses, respectively, which are acquired by contact with contaminated blood (like needlestick injury, blood transfusion, injection drug abuser), sexual contact with infected person and from mother to child during childbirth.
  3. Non-alcoholic fatty liver disease (NAFLD): Caused by presence of extra fat in liver (after excluding significant alcohol intake) which causes inflammation and scarring leading to cirrhosis. Usually seen in patients who are overweight or obese, having diabetes, high fat in blood, high blood pressure or metabolic syndrome.
  4. Drugs causing liver injury.
  5. Others: Autoimmune hepatitis (body`s own immunity acting against liver), Wilson disease (build up of copper in body), hemochromatosis (excess of iron in body), certain bile duct disorders, etc.

What are the symptoms of cirrhosis?

  1. Early disease can present with nonspecific symptoms like weakness, fatigue (loss of energy), decrease wish to take food, vomiting or nausea, weight loss, etc.
  2. Advanced disease can present with fluid accumulation in legs (edema) or abdomen (ascites), facial puffiness, yellowish discoloration of eyes or urine (jaundice), redness of palm (palmer erythema), decrease urine output, small red lesions on skin, easy bleeding following trauma, blood coming out of mouth/anus/other body site, black colored feaces, mental confusion, flapping tremors of hand, etc. In men it can cause impotence, breast enlargement and shrinking of testis.

What are the complication of cirrhosis?

Portal hypertension: It is a common complication of cirrhosis which is due to increased pressure in portal vein. Portal vein is main blood vessel which carries blood to liver from stomach, intestines, spleen, gallbladder and pancreas. Because of scar formation in liver the normal flow of blood from these organs to liver is hampered. As a result of blockage of blood flow to liver there are few complications which can arise like accumulation of fluid in abdomen (ascites) and legs (edema), formation of enlarged veins (varices) in food pipe (esophagus), stomach, etc., enlargement of spleen (splenomegaly), mental confusion/altered behavior/altered sensorium (hepatic encephalopathy), respiratory discomfort (hepatic hydrothorax or hepato-pulmonary syndrome) or decrease urine output/rise in creatinine (hepato-renal syndrome).

  1. Increase risk of infection because of immune system dysfunction.
  2. Development of liver cancer (Hepatocellular carcinoma)
  3. Easy brusibility or bleeding following light trauma.
  4. Gall bladder stone formation
  5. Metabolic bone disease

What are the stages of cirrhosis?

  1. Compensated cirrhosis: Liver damage is damaged but no abdominal swelling (ascites), blood in vomiting or black/red color feaces (variceal bleed), altered behavior/sensorium (hepatic encephalopathy), respiratory discomfort (hepatic hydrothorax or hepato-pulmonary syndrome) or decrease urine output/rise in creatinine (hepato-renal syndrome).
  2. Decompensated cirrhosis: Presence of abdominal swelling (ascites), blood in vomiting or black/red color feaces (variceal bleed), altered behavior/sensorium (hepatic encephalopathy), respiratory discomfort (hepatic hydrothorax or hepato-pulmonary syndrome) or decrease urine output/rise in creatinine (hepato-renal syndrome).

How cirrhosis is diagnosed?

  1. Your doctor will take good history and do proper physical examination. If there is suspicion of cirrhosis then he will subject you to some blood test, ultrasound abdomen, fibroscan/elastography, upper GI endoscopy and if needed CT scan or MRI of abdomen or liver biopsy.

  2. Ultrasound of abdomen in cirrhosis may shows coarse liver echotexture, nodular liver surface, dilated portal vein or collateral, enlarged spleen or abdominal fluid (ascites).

How to prevent cirrhosis?

Best way to avoid development of cirrhosis from predisposing stage of liver illness is to recognize and treat early. Few advices to keep liver healthy are:

  1. Dietary modification: Eat healthy balanced diet. Avoid high calorie food or drinks, saturated fat, sugar and refined carbohydrates. Keep yourself hydrated.
  2. Lifestyle modification: Maintain healthy body weight. Avoid being overweight or obese. Aerobic exercise (like brisk walk 30-45min/day atleast 5days/week) regularly helps to lower liver fat.
  3. Avoid: Avoid use of contaminated needles, sharing of items of personal hygiene (like shaving razors, toothbrush, nail clippers, etc), use of illicit drugs, self medication with over the counter drugs or using drugs beyond doctor`s advice.
    1. Practice safe protected sex
    2. Alcohol
  4. Motivate others: to stop drinking and follow hand hygiene and doctor`s advice.
  5. Hand hygiene: Wash hand with soap and water regularly before eating, after going to toilet and after touching dirty objects.
  6. Vaccination: For hepatitis A or hepatitis B, if you are not vaccinated or not already infected or unsafe antibody titre. Transmission rate of hepatitis B from mother to child at birth can be reduced with vaccination and immunization of newborn starting within 12 hours of birth as well as by using antiviral drugs (if indicated).

Prevent others from getting infected from you if you harbor virus causing liver damage.

How to treat cirrhosis?

Treatment of cirrhosis is based on cause of cirrhosis and complication of cirrhosis. Main aim of treatment in early stage of cirrhosis is to slow the progression of cirrhosis and prevent complications development

  1. Stop alcohol, antiviral drugs for hepatitis b or hepatitis C, proper sugar level if diabetes, steroid or other medications for autoimmune hepatitis, medications to reduce copper from body in patients with Wilson`s disease, etc.
  2. Drugs to lower portal pressure (beta-blocker or nitrates), drugs to remove fluid from body (diuretics), drugs to lower ammonia level for encephalopathy (lactulose and others), drugs to improve kidney function (albumin, terlipressin and others).
  3. Opt for liver ransplant, If cirrhosis is life threatening or treatment of cirrhosis complication is ineffective.
  4. Low salt high protein diet
  5. Stop alcohol intake even if you have other cause of cirrhosis.
  6. Stop smoking
  7. Avoid over the counter drugs (especially pain killers)
  8. Vaccinate if not done already or infected

Talayyuf - E - Kabid Or Cirrhosis Of The Liver - Know More About It!

It is a slowly and gradually progressing disease in which healthy liver tissue is replaced with scar tissue, hence preventing the liver from proper functioning. The developed scar tissue blocks the flow of blood through the liver and slows the processing of nutrients, hormones, drugs, and naturally produced toxins. It also slows the production of proteins and other substances made by the liver. Cirrhosis is a complication of liver disease which involves loss of liver cells and irreversible scarring of the liver. In other words, Cirrhosis is a condition in which the liver does not function properly due to long-term damage. Typically, the disease comes over months or years.

Although there are many other causes for cirrhosis, the commonest is excessive use of alcohol, viral hepatitis B and C infections and non-alcoholic fatty liver disease. Thus the scarring is most often caused by long-term exposure to toxins such as alcohol or viral infections.

According to the National Institutes of Health, it is the 12th leading cause of death. Men are more prone to develop cirrhosis than women. Globally approximately 57% of cirrhosis is attributable to either hepatitis B (30%) or hepatitis C (27%).Alcohol consumption accounting for about 20% of cirrhosis.

Persons most commonly affected: Adults of both sexes.

Unani approach to Cirrhosis of Liver (Talayyuf-e- Kabid)-

In Unani literature Cirrhosis of the liver is known as “Talayyuf-e- kabid”, “Sighr-al- kabid” or “Jigarki laghari”. It is of two types.

1. AcuteType of Cirrhosis of Liver (Talayyuf-e- Kabid Hadd)-

It is rare but very dangerous. It is also called Yarqaan-e- khabees or Huzaal-e- asfar. Mainly occurs due to inflammation in hepatic cells, in which hepatic cells squeeze in few days resulting in improper functioning of liver. It is a disease in which cells throughout the liver are destroyed. Although they are replaced by areas containing normal new cells but also by much scar tissue, which alters the structure of the liver.

Causes and risk factors: Excessive use of alcohol/ chronic alcoholism, poor nutrition, highly toxic condition of the body system, viral infections caused by chronic viral hepatitis (types A, B and C), metabolic diseases such as alpha-1-antitrypsin deficiency, galactosemia and glycogen storage disorders,inherited diseases such as Wilson's disease and hemochromatosis, biliary cirrhosis, toxic hepatitis due to severe reactions to prescription drugs or prolonged exposure to environmental toxins and repeated bouts of heart failure with liver congestion etc.

Signs and Symptoms: The main signs and symptoms of this type are jaundice, pain at the site of liver, vomiting, headache, dizziness, increased body temperature, increased pulse rate, brownish coloured and dry tongue, increased thirst, initially whitish coloured diarrhoea, later on, colour becomes blackish due to contamination of blood, urine passes in less quantity and reddish in colour due to contamination of yellow bile in it,  haemorrhage from nose, stomach, intestine and uterus even abortion in women are seen. The size of the liver feels small on palpation.

2. Chronic type of Cirrhosis of Liver (Talayyuf-e- KabidMuzmin)

This type of the disease occurs due to the improper blood supply to the hepatic cells resulting in the size of liver very small in appearance but liver cells stay normal.

Causes and risk factors: The main causes of this type are the application of permanent pressure at the site of the liver such as ascites, alcohol abuse, hepatitis C virus infection, cardiomegaly, poor nutrition, highly toxic condition of the body system, peritoneal inflammation, improper diet in old age and starvation etc. Other causes which develop this disease are, obstruction of the external bile duct by stone, scar, inborn defect or tumour, use of certain medications, syphilis and passive liver congestion. Obesity has recently been recognized as a risk factor in non-alcoholic hepatitis and cirrhosis.

Signs and Symptoms: It is a slowly progressing disease. In early-stage symptoms, of indigestion like flatulence, constipation and diarrhoea may be present. General weakness, the feeling of being unwell, loss of appetite, loss of weight, and a loss of interest in sex may also be there. There may be a dull abdominal ache, nausea and vomiting. Eventually, the patient dies due to anaemia or ascites. The feeling of dullness is the peculiar sign of the disease. Other symptoms and signs include low-grade fever, bad breath, jaundiced skin and bloated and swollen abdomen.

Complications and Risks associated with Cirrhosis of Liver (Talayyuf-e-Kabid)-

Ascites (retention of fluid in the abdominal cavity) is the most common complication of cirrhosis and is associated with a poor quality of life, increased risk of infection and a poor long-term outcome. Other potentially life-threatening complications are hepatic encephalopathy (confusion and coma) and bleeding from oesophageal varices. Cirrhosis is generally irreversible, and treatment usually focuses on preventing progression and complications. In advanced stages of cirrhosis, the only option is a liver transplant.

Preventive measures for cirrhotic patients (Talayyuf-e- Kabid ke Mareezon ke liye Ehtiyaati Tadaabir):

  • Cirrhotic patients must completely avoid alcohol. Alcohol accelerates liver failure and hastens death in cirrhotic patients.
  • Avoid liver toxins. Acetaminophen (Tylenol), should be avoided because they can be toxic in cirrhotic patients.
  • Juice therapy helps the liver to detoxify toxins from the body. Cirrhotic patients should mix one part of pure juice with one part of water before drinking.
  • Cirrhotic patients are strongly advised to eat smaller meals. To avoid overworking the liver, five or six smaller, lighter meals per day are recommended.
  • Avoid fatty foods and processed foods, especially foods prepared with animal fats or hydrogenated oils. These types of foods put additional load on the liver.
  • Cirrhotic patients are also advised to eat only lean proteins (containing no fats) and in limited amounts.
  • Vegetable proteins are very beneficial in Cirrhotic patients. High protein intake causes increases of ammonia levels in the blood, possibly resulting in mental confusion, and in severe cases, coma. However, do not severely limit protein intake, as this may cause protein deficiency and impair the healing process.
  • Routinely increase consumption of fruits and steamed vegetables as fruits and vegetables are easy to digest, thus less work for the liver. In addition, they are good sources of vitamins, minerals, and antioxidants that help the liver detoxify and heal.
  • Cirrhotic patients are advised to practice intermittent fasting.
  • They are also advised to protect against hepatitis B, C and E.

Fundamentals of Management in Unani Medicine-

  • Identify the cause of the disease and treat it accordingly
  • Correct the digestive processes
  • Use hot dresses
  • Use mild and early digestible diet
  • In case of ascites treat it

Management/ Treatment-

In Unani medicine, very effective treatment is available for the Cirrhosis of Liver by strengthening the Liver cells and restoring normal size and functions of the Liver. In Unani medicine, there is a number of hepatoprotective drugs that protect the Liver either acute or chronic type.

  • SootshekharRas Swarna yukta 125 Mg twice a day
  • PittariAvleh 10 gm twice a Day

Duration of the treatment: 3 to 6 months or it depends upon the condition of the patients. If the patients with cirrhosis have diabetes, then the treatment may take long time for curing the disease. Diet plays a key role along with Unani Medicines. Hence, it is strictly advised to follow a proper diet plan.
