Dear Sir, I am aged-45, Female, Weight-29 kg, from Bangladesh. I am suffering from MCTD Disease from 1999. At present I am under supervision of two doctors: One doctor is from Kolkata India, Dr. Asim Kumar Pal, MD, FICA (USA), MRCP (UK), FRCP (Edin), FRCP (London). Peerless Hospital & B.K.Roy Research Center Kolkata India, Since 1999 every year 3 times, and another doctor is from Bangladesh Professor Dr. Syed Atiqul Haq MBBS, FRCP, FCPS, MD, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Since 2006 every year 3 times. I am taking medicine methotrexate weekly since 1999 but last two years I am not taking this medicine. I also take Nidipin SR. Apart from these two medicine. I take some other medicine for short period of time recommended by my doctors. I also took steroid (Prednisone) medicine for last 9 years but not regularly. I took this medicine once in every four/six month. As a side effect of this medicine my femoral head’s blood supply stops and doctor suggested me for surgery. I did this in Apollo hospitals, Delhi. In every three months I do some diagnostic test (TC, DC ESR, AST, SGOT, Urine R/A, Chest X ray PA view, etc ….)all the reports are normal. At Present my problem are: •I can’t eat anything normally. If I try to eat it cramp in my stomach and starts beating. •Fever at least ¾ time in every month. •Whole body pain.\ •I Can’t grip my hand. •I Can’t seat alone on the floor for last 5 years. •Gastro problem. •Always cough Can I have treatment in your hospital? I request your suggestion.

no it seems like a rhumatological problem. you are in good hands. please follow the medications prescribed by yout doctors. also discuss with doctors if newer biologicals can be used.

ANA is a type of antibody test usually positive in auto immune disorders some types of arthritis, vasculitis etc. But the titre of the test makes it significant. It can be positive in low titres like 1: 40 or 1: 80 in the general population without any disease as well. So the report has to be clinically corelated and cause for positivity has to be found out to treat it.

In ITP, the body's immune system, for unknown reasons, destroys healthy blood platelets. It also impairs the body's ability to produce platelets. Slowing the destruction of platelets is one approach to ITP treatment. Most treatments for ITP interfere with the immune system to help reduce platelet destruction. Some of these treatments are FDA approved for ITP and some are not. Many of these treatments are immunosuppressants. Corticosteroids: Often the first treatment a patient receives, corticosteroids raise platelet counts by slowing the immune system's ability to clear platelets from the bloodstream. 5 In fact, a positive response to corticosteroids can be used to confirm the diagnosis of ITP. A panel of ITP experts recently suggested that patients should only be on corticosteroids for 4 weeks as patients often experience complications because of their corticosteroids medicines over time. 4 Anti-D and IVIG: These blood products include antibodies that are made from human plasma. They slow the clearance of platelets from the bloodstream tend to be used when platelet counts are dangerously low, and have an effect that is usually temporary. Anti-CD20: Treatment with antibodies that bind to the CD20 antigen on malignant human B cells marks these cells for destruction by the immune system. Several other medications are used to treat ITP, including other immunosuppressants, hormone therapies, chemotherapeutics, and biologics. Medical treatments that increase platelet production In recent years, it has become clearer that there is an additional process involved in ITP: The body doesn’t produce enough platelets to make up for the healthy platelets that are destroyed. An alternative approach works to increase platelet production supporting a normal process in the body. Platelet boosters. These are medicines that do not work on the immune system, but instead imitate thrombopoietin, or TPO. TPO is the protein that tells special cells in the bone marrow called megakaryocytes to produce platelets. By doing this, platelet boosters help increase platelet production. Hope this helps.

Take cap rabemacdsr once daily morning before breakfast empty stomach for 10 days. Take tab cefolac-- o 200 + tab crocin advance twice daily for 10 days. Take tab lumerax 80 mg twice daily for 3 days with milk after meals take syrup ferikind -- m 2 tsf twice daily after meals for 15 days.

I am 68 years old and was admitted in the hospital for 4 days for urinary track infection, Early Sepsis HTN Post TURP Thrombocytopenia. My platelets came down to 84000. Then by taking Medicines Inj. Geret 1gm once a day for 7 days and good food and rest the platelets improved to 132000. Last month it came down to 112000 and yesterday test shows 840000. What I should take to improve my platelets. The question is not of increasing platelets but of finding out the reason of low platelet count. It can be due to infections, medicines and some other conditions. Consult me with more clinical details and investigations if available to find the reason of low platelet count.
