Everolimus Test

Everolimus Test

also known as: Zortress, Certican, Afinitor

Everolimus is a drug used for curing many diseases, but mainly it acts as an antineoplastic chemotherapy drug. It has antitumor efficacy for various cancers. The trade name for Everolimus is Afinitor. It works similarly to sirolimus as an inhibitor of mammalian target of rapamycin (mTOR). It is given to the patients during organ transplant to prevent rejection and hence it acts as an immunosuppressant. It is also used in the treatment of renal cell cancer and other tumour cells. It is marketed in USA, Europe and other countries under different trade name for curing tumours. It is also available in India under the brand name Evertor.

There are a number of patent preparations of Everolimus and its intermediate. One of the preparations is as follows: Preparing a mixture comprises of Sirolimus, 2-(t-butyldimethylsilyl) ethylene glycol, base and organic solvent. Addition of triflic anhydride to the mixture to obtain crude 40-O-[2-(t-butyldimethylsilyl) oxy] ethyl rapamycin. Treating the crude 40-O-[2-(t-butyldimethylsilyl) oxy] ethyl rapamycin to obtain Everolimus. Purifying the Everolimus.

Everolimus is used in the treatment of breast cancer in postmenopausal women. It is given to adults with neuroendocrine tumours of pancreatic, gastrointestinal and lungs. It is used in treating advanced renal cell carcinoma, where treatment with sunitinib and sorafenib were unsuccessful. It is also used for adults and pediatric with angiomyolipoma and tuberous sclerosis complex. It is used during heart and liver transplantations. It is used in increasing the lifespan of model organisms like mice. It enhances the response of influenza vaccine drug in elders.

Everolimus is given as an oral intake (Tablet). It should be taken at same time every day. For e.g., on day 1 if it is taken at 10 AM, on day 2 it must be taken at 10 AM, and so on. It can be taken with or without taking food, but it should be followed for the remaining days. Consistency is important in this pattern. The whole tablet should be swallowed immediately with water, neither chew or crush the tablet. Everolimus is available in four doses: 2.5mg, 5mg, 7.5mg, and 10mg. The dosage is dependent on the severity of the disease and it is prescribed by the physicians. Without medical advice, it should not be taken.

Therapeutic targets vary by transplant site and institution protocol. Established ranges refer to trough (predose) concentrations.
