Prone To Fractures - Can It Be A Sign Of Osteoporosis?

MBBS, D (ORTHO), DNB (Orthopedics), MCH (Orthopedic), Spine Fellowship DePUY, Medtronics Spine Fellowship
Orthopedic Doctor, Delhi
Prone To Fractures - Can It Be A Sign Of Osteoporosis?
Your bone is a living tissue that is continuously undergoing the process of being broken and repairing. World Osteoporosis Day (WOD) takes place every year on October 20. Osteoporosis is a disorder characterized by brittle and weak bones. Osteoporosis affects one s hips, spine, and wrist, thus rendering a hunched posture and a shrunk appearance resulting from the collapsed vertebra.

Factors behind osteoporosis:

Excessive consumption of alcohol and tobacco
A sedentary lifestyle with not much physical activity
Certain medical disorders such as liver and kidney disease, cancer and arthritis
Medicines to fight cancer, seizures, gastric reflux and transplant rejection
Low estrogen count in women due to menopause (a phase wherein menstruation stops completely)
Dip in the testosterone levels in males because of age or other treatments and surgery
Treatments:

Medications such as bisphosphonates include Ibandronate, Zoledronic acid, Risedronate, and Alendronate, and other substitutes such as Teriparatide and Denosumab can be administered. However, these bring with them certain side effects such as nausea, abdominal pain, or heartburn.
In women, estrogen therapy post-menopause can help maintain bone density but at the same time, this can make one vulnerable to heart diseases, cancer or blood clotting.
Alternative medicines include Ipriflavone which is a laboratory-manufactured medicine and when combined with calcium, can prevent bone loss and acute pain.

Osteoarthristis - How To Take Care Of It?

MBBS Bachelor of Medicine and Bachelor of Surgery, MS - Orthopaedics
Orthopedic Doctor, Kolkata
Osteoarthristis - How To Take Care Of It?
Movement of the human body happens as a result of coordinated movement between the bones and the muscles. There are also cartilages that enable bones to move over and against each other, enabling body movement. Like other body parts, these are also prone to normal wear and tear. This causes bone movement to become less elastic, more rigid, and painful. This pain and inflammation of the bone joints is termed medically as osteoarthritis. It is one of the degenerative joint disease (DJD) of the bones and is a chronic condition.

Causes

As noted above, the main cause of osteoarthritis is aging. However, injuries with damage to the bones and muscles also cause osteoarthritis. With obesity on the rise, many patients are experiencing osteoarthritis as a result of increased weight gain also, as the bones are not able to take the excessive weight.

Symptoms

The most common symptoms include painful and stiff joints. When the movement becomes painful, there is lesser mobility, thereby leading to more weight gain. In severe cases, there could also be swelling of the joints. There could be a "crackling" sound that is typical of onset of osteoarthritis. The stiffness is more common during the later hours of the day and after prolonged period of inactivity, for instance, sitting for long hours.

Diagnosis

Though the symptoms associated with injury and/or old age can be quite diagnostic, a doctor can confirm with a formal checkup if it is indeed osteoarthritis. X-rays and test of the joint fluid can also be used for further confirmation.

Treatment

Osteoarthritis needs to be treated in a multi-pronged approach. As noted earlier, this condition leads to a vicious cycle leading to weight gain. Therefore, the treatment should be aimed at improving movement, reducing pain, reducing weight, and adapting a healthier lifestyle. Also, though it may not be completely prevented, the onset can be delayed by choosing a healthier lifestyle during the younger years of life. Exercise and weight control can definitely prolong the onset of the condition.

There is a strong genetic component that is mapped to this condition. So, if you are aware of an existing family history, it should trigger you to undertake preventive measures earlier in life. Adapting a healthy lifestyle definitely can be helpful in numerous way, prolonging and reducing the severity of osteoarthritis being one of them.

Stress Fracture - How To Avert It?

MBBS Bachelor of Medicine and Bachelor of Surgery, MS - Orthopedics, Fellowship, Depuy Joint Replacement
Orthopedic Doctor, Bhubaneswar
Stress Fracture - How To Avert It?
One of the most common injuries in sports is a stress fracture. Overcoming an injury like a stress fracture can be difficult, but it can be done.

A stress fracture is an overuse injury. It occurs when muscles become fatigued and are unable to absorb added shock. Eventually, the fatigued muscle transfers the overload of stress to the bone causing a tiny crack called a stress fracture.

Causes :

Stress fractures often are the result of increasing the amount or intensity of an activity too rapidly. They also can be caused by the impact of an unfamiliar surface (a tennis player who has switched surfaces from a soft clay court to a hard court); improper equipment (a runner using worn or less flexible shoes); and increased physical stress (a basketball player who has had a substantial increase in playing time).

Common sites:

Most stress fractures occur in the weight-bearing bones of the lower leg and the foot. More than 50 percent of all stress fractures occur in the lower leg. The repetitive stress of the foot striking the ground can cause trauma. Without sufficient rest between workouts or competitions, an athlete is at risk for developing a stress fracture.

Stress fractures affect people of all ages who participate in repetitive sporting activities, like running. Medical studies have shown that female athletes seem to experience more stress fractures than their male counterparts. Many orthopaedic surgeons attribute this to a condition referred to as "the female athlete triad", eating disorders (bulimia or anorexia), amenorrhea (infrequent menstrual cycle), and osteoporosis. As a female's bone mass decreases, her chances of getting a stress fracture increases.

Presenting complaints:

Pain with activity is the most common complaint with a stress fracture. This pain subsides with rest.

Diagnosis:

X-rays are commonly used to determine stress fracture. Sometimes, the stress fracture cannot be seen on regular x-rays or will not show up for several weeks after the pain starts. Occasionally, a computed tomography (CT) scan or magnetic resonance imaging (MRI) will be necessary.

Treatment:

The most important treatment is rest. Individuals need to rest from the activity that caused the stress fracture, and engage in a pain-free activity during the six to eight weeks it takes most stress fractures to heal.If the activity that caused the stress fracture is resumed too quickly, larger, harder-to-heal stress fractures can develop. Re-injury also could lead to chronic problems where the stress fracture might never heal properly. In addition to rest, shoe inserts or braces may be used to help these injuries heal.

Prevention:

1. When participating in any new sports activity, set incremental goals. For example, do not immediately set out to run five miles a day; instead, gradually increase distance.

2. Cross-training - Alternating activities that accomplish the same fitness goals -- can help to prevent injuries like stress fractures. Instead of running every day to meet cardiovascular goals, run on even days and bike on odd days. Add some strength training and flexibility exercises to the mix for the most benefit.

3. Maintain a healthy diet. Make sure you incorporate calcium- and Vitamin D-rich foods in your meals.

4. Use the proper equipment. Do not wear old or worn running shoes.

5. If pain or swelling occurs, immediately stop the activity and rest for a few days. If continued pain persists, see an orthopaedic surgeon.

6. It is important to remember that if you recognize the symptoms early and treat them appropriately, you can return to sports at your normal playing level.

Pain & Tenderness In The Hand Bones - Know The Possible Reasons!

MBBS, MS - Orthopaedics
Orthopedic Doctor, Jodhpur
Pain & Tenderness In The Hand Bones - Know The Possible Reasons!
Are you experiencing pain and tenderness in your hand bones? Hand pain can occur due to several causes that may include diseases and injuries. Most of the causes of pain in the hand bones are treatable, and the symptoms can be eased. The most common conditions that cause pain and tenderness in your hand bones include the following:

De Quervain s tendinitis

This condition causes pain on your wrist on the side of your thumb. The pain develops gradually or suddenly and may travel the length of your thumb and forearm.
Movements such as making a fist, turning the wrist, and holding objects may be painful.
The pain occurs because of swelling of your wrist tendons that are present at the base of your thumb. This happens on the account of inflammation and irritation.
Overuse and repetitive activities lead to this condition.
New mothers are at a higher risk of getting this condition because of the position in which they have to hold their babies, and due to fluctuating hormone levels.
Wrist fracture increases the chances of De Quervain s tendinitis.
Arthritis

Arthritis is a very common condition that is associated with hand pain or pain in the hand bones.
It causes your joints to lose the cartilage, slowing them to move against each other slowly.
The deterioration of the cartilage causes pain and swellings.
The areas in your hand that get affected by this condition include the base of your thumb, end joint located near your fingertip, and the middle joint of your fingers.
The most common form of arthritis is known as osteoarthritis, which leads to the progressive degeneration of the cartilage. It occurs because of ageing, and because of injuries such as dislocation and fractures. It causes swelling, stiffness, and swelling in your hands.
Another form of arthritis that affects your hand bones is called rheumatoid arthritis. This is an autoimmune disease in which the cells lining your joints get attacked by mistake by your own immune system. It leads to stiff, swollen, and painful joints. Hand pain associated with this condition is aching and throbbing in nature. The pain worsens in the morning and after a period of being immobile.
A Ganglion

This is a fluid-filled swelling which develops around a tendon or joint. A ganglion resembles a soft and smooth lump.
It is made of synovial fluid which surrounds your tendons and joints for lubrication during movement.
These are most likely to occur in your wrists, fingers, and hands.
It is recommended for you to consult a doctor on experiencing hand pain of any form, and at any location on the hands. This will allow proper diagnosis so that you can follow specific treatment measures.

Running Injuries - Know Forms Of Them!

Running Injuries - Know Forms Of Them!
Too much of something is never a good idea. Running is a great way to exercise, but if you don't run in moderation and do not take care of yourself, you could foresee more than one health problem. If you push yourself too hard as a runner, you could face one or more of these running problems.

Runner's knee: If the bones in your knee shift out of alignment, the condition is called runner's knee. This is one of the most common overuse injuries. At first, you may not realize it, but with time, this can cause the cartilage in the knee to wear out. This can create excruciating pain. A few simple ways of making yourself more comfortable are:
Squatting
Walking up and down stairs
Bending your knee while sitting
Stress fracture: A stress fracture is a small crack in the shin bone or one of the bones in your foot. This can happen if you suddenly over exert without giving your body time to get used to running. Since it is only a crack, a doctor will not normally out your leg or foot in a cast. The only way to let your bones heal in such a condition is to rest it.
Shin splint: Flat-footed people are most susceptible to shin splints. Shin splints can be caused by stress fractures and overused or weakened muscles. To treat a shin splint, give your leg plenty of rest and gently stretch it. Give yourself a few weeks to recover before you start running again.
Achilles tendinitis: Inflammation of the Achilles tendon is known as Achilles tendinitis. The Achilles tendon attaches the calf muscle to the heel. Suddenly increasing the distance you run in a day is the most common cause of Achilles tendinitis. The most common symptom of this condition is pain and stiffness around the ankle. Ice the area to deal with the immediate pain and discomfort. Your ankle will also need plenty of rest to properly heal. Stretching the calf muscles can also prove to be beneficial.
Muscle pulls: Almost every runner suffers from a muscle pull at some point or the other. A muscle pull can be defined as a small tear in the muscle. Muscle pulls usually affect the hamstrings, quadriceps, calf and groin. To treat a muscle tear, keep your leg elevated; give it plenty of rest and use an ice pack or compression pack on the area.
Apart from these, a few other injuries that runners often suffer from include ankle sprains, blisters, IT band syndrome and plantar fasciitis.

Braces For Osteoarthritis Knee - In Depth!

MBBS, Diploma in Orthopaedics, M.Ch - Orthopaedics
Orthopedic Doctor, Gurgaon
Braces For Osteoarthritis Knee - In Depth!
Osteoarthritis may affect any joint of your body. And knee arthritis is one of the most common versions of arthritis. It can cause immense inconvenience and braces can help mitigate many of these bothers.

When Do Braces become a Necessity?

The cartilage at the knee joint cushions the knee and protects the knee from the shock waves of walking, running and other movements by absorbing them. But in some people, the cartilage begins to wear out slowly and when this happens, the underlying bone is exposed and made vulnerable. The degeneration of the cartilage and its consequences trigger excruciating pain, disruption of weight distribution and the unnatural growth of bone spurs around the knee joint.

As the disorder progresses, one side of the knee wears away more than the other and this affects the person s gait as well. To correct the misalignment, doctors prescribe knee braces.

How Does A Knee Brace Work?

A knee brace is like a tightly fitted sock that you slip your leg through and pull it up to the knee. It sits snugly around your knee. It exerts pressure on your knee and the surrounding area.

A knee brace redistributes the weight evenly. It shifts the weight from the worn and damaged part of your knee onto a lesser damaged part. That eases a lot of the pain you had been suffering. A knee brace will help you get back on your feet and even walk long distances. It puts to rights your gait and stance as well.

Are there any Complications that can arise out of Knee Braces?

Knee braces may help you return to a normal life, you must be prepared for a few minor problems.

Discomfort -

You will feel uncomfortable until you get accustomed to the braces. The knee and the surrounding areas might itch and become sore. The brace will feel cumbersome and your knee might even feel hot and sweaty.

The knee skin may become red and swell up -

The only way for braces to be effective is if they fit well. Your doctor can help recommend the brand and size. Moreover, braces are made with different kinds of materials. You need one that suits your skin. After you get the brace fitted to your knee, visit the doctor again who will be able to tell you if the brace is indeed perfect for you.

Unicondylar Knee Replacement - Who Can Opt It?

MBBS, MS - Orthopaedics
Orthopedic Doctor, Ahmedabad
Unicondylar Knee Replacement - Who Can Opt It?
Knee problems can be quite annoying but if this problem becomes regular, it brings life to a standstill. The movement becomes restricted. Many associated problems slowly crop up. Thus, it is best to nip the problem in the bud. While some people might experience a problem in the full knee, in some people, only a single compartment (tissue or cartilage) of the knee may be affected. For such patients, Unicondylar Knee Replacement comes as a blessing.

The Unicondylar knee replacement, also known as the Unicompartmental Knee Replacement or Partial Knee Replacement is the most sought-after knee replacement surgery. The surgery involves replacing only the worn out or affected part (cartilage) of the knee, thereby preserving the healthier cartilages and tissues of the knee.

In the last few years, incidences of osteoarthritis have been on the rise. The unicondylar knee replacement surgery has made life a lot easier for people suffering from osteoarthritis (a condition where the articular cartilage or the connective tissue present within the knee joint begins to wear out).

The unicondylar knee replacement is ideal for people with:

Severe osteoarthritis (Median or Lateral) that results in painful, swollen and stiffened knee.
Only a small portion of the knee is affected.
A person experiences great difficulty in movement.
People over 48 years of age are mostly advised to undergo this surgery. Many young people with osteoarthritis opt for this surgery.
However, the surgery may not be a wise idea for

A person with Rheumatoid Arthritis and Angular Deformity (acute).
A person whose larger portion of the knee is affected.
A person who had undergone osteotomy (surgical excision followed by reshaping of bones).
People with an unstable or weak knee should avoid this surgery.
Pre- surgery, a person should

Avoid taking any anti-inflammatory and herbal medicines (minimum 10 days before the surgery).
Get yourself medically examined for any health complications (that might interfere with surgery).
Follow a healthy lifestyle. Avoid smoking and drinking.
Procedure:

For the surgery, the patient is either given a general anesthesia or a spinal (or epidural) one.
Next, a compressing device (tourniquet) is put around the upper part of the thigh. This is done to avoid excessive blood loss during the surgery.
A 7cm incision is made over the knee.
The damaged parts (bones and cartilages) of the knee are then carefully removed.
The surgeon next replaces the excised parts with metallic implants. Once the metallic part fits into the knee, it is adhered to the bone with (or without) bone cement.
The surgeon then stitches the incised area, followed by dressing and bandage.
The recovery time is quick, between 3-4 weeks. However, the concerned person may require physiotherapy for the next 3-6 months.

Advantages:

The surgery is less invasive with a quick recovery time.
The surgery replaces only the affected knee part.
Blood transfusions are seldom required.
A person gets back to normal life faster.
The surgery requires less time (~ 1-2 hours).
You need to be careful about:

Infection at the incision site, though it is very rare.
Injury in blood vessel, ligaments, or nerves.
Fracture in the bone

Osteoporosis & Osteomalacia - How Distinct Are They?

MBBS, DNB - Orthopedics/Orthopedic Surgery, Fellowship in Minimally Invasive Knee and Hip Replacement, Fellowship in Joint Replacement & Arthroscopy
Orthopedic Doctor, Pune
Osteoporosis & Osteomalacia - How Distinct Are They?
Osteoporosis:

Osteoporosis is a condition whereby the density of the bone decreases and the body ceases to manufacture the same quantity of bone as it did previously. Although it tends to affect both females as well as males, it is most commonly seen in women. The reason for this is the decreased production of estrogen during the menopause phase. The hormone, estrogen provides protection against osteoporosis.

When someone suffers from this condition, all their bones become weak and they become highly susceptible to fractures in the event of an accident or even a slight knock.

The word osteoporosis means porous bones. Once a person crosses the age of 35 years the rate of bone renewal or rebuilding slows down. When this happens on a prolonged basis the concerned person is highly prone to develop osteoporosis.

Key Points:

Those who smoke and have a poor diet are more likely to suffer from osteoporosis.
Osteoporosis targets areas of the body such as the hip, wrists and spine.
Although there is no particular outward sign of osteoporosis you are likely to develop a stoop as a result of a weak spine. This will further lead to bone pain.

Osteomalacia:

Osteomalacia is a condition that is similar to osteoporosis where the bones are weakened. But what makes this condition different from osteoporosis is that in osteomalacia there is difficulty in the process of bone-building. In osteoporosis, existing bones become weak.

Reasons for Osteomalacia:

Osteomalacia occurs when there is vitamin D deficiency. As a nutrient, the benefits of vitamin D are enormous because it is instrumental for the absorption of calcium in the stomach.

Vitamin D is helpful for the maintenance of phosphate and calcium levels which is needed in the proper formation of bones. Vitamin D can be acquired from ultraviolet rays of the sun and also from food items such as fish and dairy products.

Signs of Osteomalacia:

Signs of osteomalacia are few. The most common signs include fracturing of the bones and weakening of the muscles. A person who suffers from the condition of osteomalacia has a difficult time walking and may develop abnormal gait.

Treatment of Both Osteoporosis and Osteomalacia:

When it comes to the treating osteoporosis it is mainly focussed on preventing the onset, maintaining healthy bone density, preventing fractures and reducing pain. This treatment is done by making adjustments to your lifestyle as we daily meal plan in addition to taking medications and health supplements.

As the first line of treatment of osteomalacia, the patient will be prescribed a number of phosphate, vitamin D and calcium health supplements. Additionally, the patient will be asked to spend some time out in the sun for the absorption of sunlight.
