About Telma H Tablet

This medication is used for treating high blood pressure and acute hypotension. It may also be used for kidney disorders. The medicine contains Telmisartan and Hydrochlorothiazide as its primary ingredients.

The contraindications to the medicine may include hypersensitivity to it. Other contraindications can be kidney diseases, liver diseases, diabetes, asthma, allergic reactions to the medicine, patients having high cholesterol level and obstructive biliary disorders.

The medicine has certain side effects. The most common effects are muscle pain and muscle cramps, allergic reactions to the medicine, drowsiness or dizziness, electrolyte imbalance and patients may face swelling on their face and lips.

The duration of the effect of the medicine lasts almost 24 hours after consumption. The onset of action of the medicine is observed within 2 hours of consumption. The medicine is not reported to be habit-forming or addictive.

The medicine is not considered safe with alcohol as it may cause drowsiness and fainting. It is not considered safe for pregnant and breastfeeding women. The drug should be taken under medical supervision. It is not recommended for patients having severe kidney disease and liver diseases.

Take exactly as directed. Do not miss doses, alter dosage, or discontinue without consulting prescriber. Do not alter salt or potassium intake without consulting prescriber. Monitor blood pressure on a regular basis as recommended by prescriber; at the same time each day.

You may experience postural hypotension (change position slowly when rising from sitting or lying, when climbing stairs, or bending over); or transient nervousness, headache, or dizziness (use caution when driving or engaging in tasks requiring alertness until response to drug is known).

Report unusual weight gain or swelling of ankles and hands; swelling of face, lips, throat, or tongue; persistent fatigue; dry cough or difficulty breathing; palpitations or chest pain; CNS changes; gastrointestinal disturbances; muscle or bone pain, cramping, or tremors; change in urinary pattern; or changes in hearing or vision.

    • Are there any missed dose instructions?

      The missed dose must be taken as soon as possible. If the time for the next dose arrives, skip the missed dose and resume the normal schedule. Do not take extra dosage.

    How does this medication work?

    This medication displaces angiotensin II with very high affinity from its binding site at the AT1 receptor subtype, which is responsible for the known actions of angiotensin II. It also inhibits a sodium-chloride symporter SLC12A3 in the distal convoluted tubule to inhibit water reabsorption in the nephron. The symporter reabsorbs sodium and creates an osmotic gradient for water to be reabsorbed, inhibition of which prevents water reabsorption.

      • Interaction with Alcohol

        The medicine may interact with alcohol and cause drowsiness and fainting. Activities that require high mental alertness are not advised for the patients.

      • Interaction with Medicine

        • Digoxin levels may be increased.
        • Lithium: Risk of toxicity may be increased by telmisartan; monitor lithium levels.
        • Potassium-sparing diuretics (amiloride, potassium, spironolactone, triamterene): Increased risk of hyperkalemia.
        • Potassium supplements may increase the risk of hyperkalemia.
        • Trimethoprim (high dose) may increase the risk of hyperkalemia.
        • Warfarin serum concentrations may be decreased (not associated with alteration in INR); monitor INR closely.
      • Interaction with Disease

        The medicine’s interaction with diabetes is moderate as it can affect the blood glucose levels. Patients having congestive heart failure must take it with caution as it can worsen their conditions and renal impairment. Patients with low sodium levels must be alert as the medicine can cause hypotension.

        The medicine might interact with foods containing high amounts of potassium as it can cause fatigue, irregular heartbeat, and mental confusion. The unusual symptoms must be reported to the doctor.
