Exclusive breast feeding up to 6 months of age is best. Since your wife has breast milk though rarely, she should continue breast feeding till her breast is emptied and then give top milk which should be kept ready before breast feed. Repeat it every time, she feeds baby. I am sure, breast milk shall be sufficiently produce after some time.
No baby food it’s only a myth. Try to breast fed 3-4 times in night which will help to produce enough amount of milk at day time for baby due to hormonal action. Baby should not drink water even.

Your baby is not gaining wt properly. By the age of 6 months weaning must have started. Now continue your feed and start giving dairy milk, cereals like thin suji kheer, khichadi, mashed vegetables, dal adding oil or ghee etc. start with one item once or twice in a day to develop taste and then increase frequency and quantity gradually. There is no need of FM.
