Treatment Of Cancer

I am Dr. Ashok Mehta, Surgical Oncology. Today I will talk about cancer. Most of the cancers are seen of mouth and throat what we call oral cancer and cancer of the hidden neck. Oral cancer is common because of the tobacco abuse which is so common in our country. And people are taking it with mava etc. Even pan masalas are not good for the oral cavity. The important thing when we treat oral cancer or throat cancer, we should have a good cosmetic and functional result. It is very important because it is a visible part of the human face. 2nd thing is the functional because of this we have to chew also to swallow and speak. So, all these functions must preserve and should be normal. I am going to show you some of the cancers that we have treated. How have we preserved the jaw especially the lower jaw? You will see that we have removed the teeth bearing part of the jaw. And you can see that this is what end result. Before and after surgery, almost 10 years after surgery, this lady looks almost similar. This is an excellent cosmetic and good functional result. Similarly, another patient who had the cancer of the mouth which was almost going to the upper part of the jaw. You can see that we have removed entire cancer with the upper part of the jaw and this defect has been covered with the free flap.
That means the skin flap which we have taken from the blood vessels from the thigh. Here is the end result. You can see the man almost looks normal. Here is the patient who had a very extensive cancer which involved not only the mouth but also the overline skin and the jaw. As you can see what we have removed, the extensive part of the oral cavity with the jaw and overline skin. This is called a free funeral graft. The bone from the leg is taken and shaped. It is used to replace the entire jaw and the skin as well as inside lining of the mouth. We have been able to do excellent cosmetic and functional result. This is because of teamwork. We have excellent microvascular and a plastic surgeon. We have a surgeon who helps us to realign the jaw. And an excellent cancer surgeon who does everything together. The result is that we deliver a normal patient. All these cancers with the advanced disease also require further treatment with radiation and chemotherapy but the important part is that we should do a good surgery so that the long term survival has a good survivals.
