This is Dr. Rahul Shetty, the consultant dermatologist. So, today we will be talking about acne, its various grades, the treatment and the preventive measures. So, acne usually called as pimple has a certain number of grades to it, we have grade one acne which usually presents as whiteheads or blackheads on the face what we call as comedones to grade four acne which usually presents as larger, bigger boils which are usually called as a noddle or cystic acne. So depending on the grade of acne, the treatment varies from person to person, which depends on the dermatologist treating you. Now, if you present to a dermatologist,the treatment modality is differ, in between becoming a medical treatment or a treatment which are usually done in the clinic. Now medical treatment usually involves certain creams, lotions, and certain tablets. Cream, lotion depends on the grade of acne as well as the tablets usually include oral supplements such as vitamin A supplements as well as the anti-biotic. So, the skin regimen usually involves certain components such as a foaming face wash with salicylic acid which helps you in removing the oiliness as well as exfoliating your face. After that mild moisturization should be applied which helps in sealing the pores as well as providing hydration to the skin. This has to be followed by a very light sunscreen with a proper SPF to protect your skin from the sun because acne can leave behind marks as well on your face and this can become unsightly as you go along the treatment. Lastly, a night cream usually involves a retinol based cream, it usually involves an antibiotic combination such as benzoyl peroxide or clindamycin to treat the infection which is usually associated with pimples. These can be combined with oral supplements as mentioned earlier such as vitamin E supplements, which helps in turning over the skin quickly, exfoliating the skin as well as the closing of the open pores. These can be combined with oral antibiotics, if the infection is left unchecked. So, usually, a dermatologist can also recommend certain treatments that can be done in the clinic such as chemical peeling, a chemical peeling helps in exfoliating the skin, lightening the marks by acne and also helps in tightening the skin. So, sometimes the pits left by the acne usually called as acne scars also become lesser and lesser visible. If acne scars are present that is usually the pits on the face, then you can go for certain other treatments such as derma rolling, you can go for micro-needling radiofrequency or certain other lasers can be used to help resurface the skin. Lastly, we have the preventive measures, preventive measures usually involve having a well-balanced diet and good night’s sleep, avoid popping the pimple, so that it can not leave behind any marks or spots, and consulting a dermatologist at the earliest. I have to summarise acne is and adolescent problem but can occur in adults as well. It should always be treated by a dermatologist so that you get the best results possible, it can have a high recurrence rate as it can come back again and again but consulting a dermatologist and getting it treated is the way forward. I am Dr. Rahul Shetty, MD, Dermatologist. So, today I will be talking about certain skin regimens which will be suitable for all skin types. So we have 4 basic elements that are included in all of the skin types. The first thing which comes into our skin regimen is a face wash. So a facewash should be a foaming base facewash in cases of an oily type of skin or it should be a very mild cleanser in cases of very sensitive skin. They should help in removing the dirt and grime from the face and in females they should help in removing the clurr, the makeup as well. So, micellar water can help in removing the makeup and then a person can go for more of a foaming face wash with the other element. So, the other element included in our daily skin regimen is vitamin C serum. The important thing of vitamin C serum is a concentration should ly between 10-15%. Vitamin C serum has multiple benefits as in acts as an anti-oxidant agent, act as an anti-ageing agent and it acts as an anti-pigmentation agent as well. So, it should be part of every skin regimen, moving forward to get a brighter look as well as a clearer face. The third element which comes into the picture and the most important element is a moisturizer. A moisturizer helps in hydrating your face as well as retaining the hydration within the skin. A moisturiser always depends, again on the skin type, the oily type of skin, requires a very light moisturiser or a silicon-based moisturiser whereas a very dry type of skin requires a creamy based moisturiser. There are many moisturizers in the market which combine other elements such as hyaluronic acid for anti-ageing, they even have vitamin C, E elements which help in hydrating the face as well as providing the anti-pigmentational elements. Lastly, we come to the night cream, a night cream basically depends on what indication you are presenting with, it is a dark circle, it has to be treated with an eye cream, targetting just dark circles, if it has pigmentation points, you have to go for a deep pigmenting cream, if anti-ageing is your focus then you have to go for certain creams which help in tightening the skin, which usually contains hyaluronic acid or retinol, usually a retinol based cream helps in all of these, as it helps in turning over the skin, help you heal in during the night time, a retinol based cream should be applied minimally with the p size amount taking on the finger and apply it generously around the face which can be combined with the moisturiser as well. This is Dr. Rahul Shetty MD dermatologist. Today we will be talking about pigmentation. Pigmentation is the discoloration or the discoloured patches which appear on your face. Pigmentation is usually caused because of the excess production of melanin or the lack of melanin in your skin. Excess production of melanin causes hyper pigmentation where as the lack of it causes hypo pigmentation. Today we will be discussing mainly about the hyper pigmented patches which appear on your face. Hyper pigmentation usually occurs because of the commonest cause being the sun exposure causing the Brown sunspots, melasma post-inflammatory hyper pigmentation, age spots and birthmarks. So the commonest pigmentation is because of the exposure to the sun. The sun causes something we call as photo pigmentation in the skin because it produces an excess of melanin in the skin and results in hyper pigmentary spots on your face.
So, these can appear as freckles and can range from freckles to melasma as well. Now the treatment differs according to the pigmentation present in the face. Freckles will appear as dark brown spots on your face which usually are present during summer and they fade out in winter. The treatment varies depending on the severity of the freckles and usually just sun protection will help in reducing the freckles for you. Melasma, on the other hand is a more stubborn pigmentation which is usually present on the face and usually on the butterfly area of the face ranging over the nose and the cheek area. Now this is usually present in pregnant women called as sclersma and it can persist for many years together. Now the treatment in it requires certain persistence on the part of the patient as well because it requires sun protection, a variety of treatment options such as peels, lasers and also PRP to be done. Post-inflammatory hyperpigmentation is another common cause of pigmentation. It usually appears after a certain amount of infection or inflammation which occurs in the body and which may lead to an overflow of melanin to the skin. Post-inflammatory hyperpigmentation is another common cause of pigmentation which can occur anywhere on the body.
It usually occurs after an infection or an injury which can lead to the melanin actually becoming excess in these areas. The melanin excess causes the pigmentation and that can last for at least months to years if left untreated. So the treatment options involved in these cases usually involve again a certain amount of peel sessions to be done. So, the treatment of post inflammatory hyper pigmentation usually involves a certain amount of medical treatments, it involves a certain amount of treatments done in the clinics such as peels and lasers which can help in lightening these areas. Now coming to the treatment of pigmentation, treatment of pigmentation usually requires a certain amount of persistence from the patient’s part because they have to follow a proper skin regimen on their own as well as certain treatments which can be done in the clinic. Now the treatment what the patient usually has to follow that is a skin regimen which is usually prepared by a dermatologist usually involves photo protection with the help of a sunscreen, de-pigmenting agents which can include hydroquinone and lastly they have to apply certain other natural components like vitamin C which can help in reducing the pigmentation as well.
Now coming to the treatments which are done in a clinic setup usually involves peels which can be done, they usually involves Q switched lasers and they can also involve newer technique such as resurfacing carbon dioxide lasers, pico lasers as well as platelet rich plasma can also be used to reduce the melanin content. Now peels and lasers these are the commonest things which are used to help reduce the pigmentation and when done in conjunction help in reducing the pigmentation to considerable amount. Peels help in reducing the melanin content of the skin by exfoliating the superficial layers of the skin whereas a laser can be used to reduce the melanin content targeting different layers of the skin depending on what layer the pigmentation has usually settled in. Whenever you have a pigmentation problem, always consult a dermatologist so that they can make a proper skin regimen for you and do the necessary treatment required.
