Hi,

I am Dr. Kamleshkumar Pandey, Pulmonologist. Aaj main aap ko TB ke baare mein bataunga. Kin patients ko TB hone ke jayada chances hote hain. Jin patients ka weight kam hai, low BMI, nutrition kam hai, kisi bhi patient ko chronic condition hai like respiratory disease ya fir chronic kidney disease ya fir diabetes, HIV ya fir unko immunosuppressive drugs lena padta hai jiski vajaha se unmein kamjori aati hai, toh ye ssare patients hain TB ke risk pe. Patients ko genetically bhi TB ho sakta hai. Dusra risk hai healthcare workers ko bhi TB ho sakta hai. TB 2 types ka hota hai. Pulmonary and extrapulmonary jo dusre organs ko involve kare. Diagnosis kaise hota hai. Sabse pehle chest x-ray, bulgum ki jaanch se humein pata chalta hai ki TB hai ya nahi. Bulgam ke liya 2-3 special tests hain. Nucleic acid amplification test.

Ye test humein bata deta hai ki ye TB simple hai ya fir drung resistance type ka hai. Extrapulmonary ka diagnosis kaise kiya jata hai? Ismein important hota hai cervical limbs nodes ki swelling, chhatti mein pani ho jaata hai, peat mein paani ho jaata hai, abdmonal TB ho sakta hai. In sab conditons me koshish karte hain ki fluid nikal ke testing karein. Agar fluid nahi nikal sakta toh tissue ko nikal kar testing karein. Confirm kar ke medicines start karne mein patient ke liya hamesha acha rehta hai. TB aisi bimari jismein lamba ilaaj chahiyai hota hai. Isliye humein bimari ki jitni jyada information ho, utna patient ke liya acha rehta hai. Iske baad TB ke management mein medicines. Isliye patient ko mediicnes ke baare mein doctor se ache se puchna chahiyai.

Medicines ke common side-effects bhi puch lena chahiyai. Example ke liya toilet ka color orange ho jata hai. Agar ye baat patient ko pehle se pata hogi toh vo ghabhrayega nahi. Isliye patient ko har ek baat doctor se puch leni chahiyai and agar koi side-effect aaye toh kya action lena hai vo bhi patient ko discuss kar lena chahiyai. Next important point hai ki patient ko treatement regular lena hai. Bich mein treatment band nahi karna hai. Medicines lene se patient ko bahut acha lagne lagta hai. Toh patient mein tendency ho jayegi ki vo medicines band kar sakta hai. Agar medicines band hui toh TB kharab mein convert ho sakta hai jise MDR TB kehte hain. So, important hai ki patient ko apni drug duration puri karani hai. Regular follow-up rakhna hai doctor ke sath.

Taki doctor identify kar sake ki patient mein kitna sudhar hua hai. Ek toh dekha jata hai ki taklif kam hui hai ya nahi, patient ka weight kitna badha hai kyuki ye imporvement ka sign hai, patient ke new tests ke according bimari kam ho rahi hai ya nahi. Isliye regular follow-up bahut jaruri hai. TB ke treatment ke sath patient ko healthy nutrients lene hain. Patient ka protein intake acha hona chahiyai, multivitamins lena chahiyai. TB patient ko sath mein HIV, diabetes ya fir koi aur chronic bimari ho sakti hai. So, har bimari ko TB ke time pe utna hi important lena hai jitna vo TB ke bina hoga. I am Dr. Kamleshkumar Pandey, Pulmonologist. Today I would like to talk about COPD. It is a common preventable and treatable disease which is a very common cause of modality and morbidity in our country. Now, we need to understand which patients are at the risk of COPD. Now, patients who have the risk of exposure to smoke, biomass fuel exposure and industrial pollutant exposure for a prolonged period of time. These patients are at high risk of COPD. If these patients have symptoms of chronic cough and breathlessness then they should get evaluated with spirometry. It is a simple blowing test which will help us in understanding first the diagnosis of COPD, the severity of the COPD, to access the response of the treatment. So, these patients need to undergo this test for the early diagnosis and management of COPD.

Next is we have to identify and eliminate risk factors. Many times, patients say that we already have smoked for so many years now what is the point of stopping smoking. But it is never too late to give up smoking. It is always beneficial to quit smoking at any given point in time. Patients who have industrial exposure can use mask or barrier method to avoid the exposure. Women who have the problem with cooking gas, they can convert into LPG. Next important point is to take inhalers regularly and learn the right technique to use inhalers. Many times patients have used inhalers incorrectly. They need to learn the right technique of using inhalers. Next important point is to exercise regularly. As COPD patients are very breathless, they do not prefer to do any sort of physical activity. Since they avoid the same, their muscles get further weak and these muscles need more oxygen for the efficiency.

So, we have to break this vicious cycle for not doing exercises. The patient can do simple walking 10-20 mins. That is the good form of rehabilitation. Also, patients should go proper pulmonary rehabilitations ate the centres. Next is how to prevent this problem? We need to vaccinate. Flu and pneumonia vaccination should be given to all COPD patients to prevent it. Because preventions have shown that patients improve quality of life as well as reducing the cost of COPD. Lastly, I want to say that COPD is not the death sentence.
