I am Dr. Rahul Gupta. So today the topic of discussion is reversal and prevention of heart disease through natural ways. Now we all know as the blocks become severe in the heart, the blood supply to the heart blood vessels, heart muscles decreases because of which a person can get a variety of symptoms like heaviness in the chest, burning in the chest, choking in the throat, pain in the jaw, breathlessness and extreme tiredness which comes on exertion and goes away with rest. Now there are multiple ways of treating the blockages so the most aggressive way is the bypass surgery, which is open-heart surgery. The lesser aggressive way is the angioplasty, but we also have ways of preventing and treating these heart diseases in a natural way. I have been promoting a 6E concept, which if followed properly can actually reverse heart disease. So the first E is about exercising and you have to follow kinds of exercise.

First one is the cardiovascular exercise which you do 30-40 minutes every day for at least 5 days a week like jogging, running, cycling, walking whatever suits you better. Second part of the exercise is your stretching exercise so you rotate, twist and turn all your body parts in to and fro manner so the joints are oiled well. Third part is toning so you tone all your large muscles like chest, back, thighs, biceps, triceps by using your own body weight or by using the weights in the gym. The second E is your eating habits. There are 3 things in the eating which you have to take care of. First is eat healthily, so anything which is less oily, less sugary, less salty, more of fruits, vegetables or whole fruits or whole grain products are more healthy. Second is you should eat timely in a correct proportion so breakfast should be early and high in proportion, lunch should be a little lesser and dinner should be early and the least of the proportion, then you should also eat in a peaceful way, when you are eating you should completely enjoy your food, not looking TV, talking on the phone and you should avoid that.

Third E is exit your addiction so when you exit your addictions like smoking tobacco, excess alcohol intake your cardiovascular system starts improving, passive smoking is equally dangerous so when you are in the vicinity of a smoker you are actually passively smoking and harming your body so avoid it. Fourth E is your emotional health. You should see how emotionally you are feeling, if you are feeling positive, happy, optimistic that means you are on the right track, while you are feeling negative or you are in a negative set of emotions then you are harming your cardiovascular system because the inner-cell lining endothelium gets damaged whenever you are in negative emotions which are a fertile ground for the development of blockages so be in a happy and optimistic mind. The fifth E is enough good quality sleep. So you should sleep for 6-8 hours every night and you should have a good quality of sleep, which means when you wake up you should feel fresh, those people who are not feeling fresh even after sleeping 6-8 hours they should look back and ask their other relatives whether they snore alot, if they are snoring they should consult a sleep specialist.

Similarly, sleep hygiene in the night has to be good. Sixth E is evaluating your numbers regularly. By numbers I mean, your risk factors number like your cholesterol number, sugars number, blood pressure number, weight numbers because all these risk factors are like silent killers, they will be there in the body, damaging the body and you do not come to know it unless you check it. I am Dr. Rahul Gupta. Today the topic of discussion is the latest and advanced treatment for severe blockages. Whenever a person develops a severe blockage. He develops multiple symptoms. The symptoms could be chest pain, burning, choking in the throat which comes on exertion and goes away with rest. Similarly, breathlessness, tiredness, extreme exhaustion. The diagnosis is done through angiography and the angiography tells us where these severe blockages are located. Now once the severe blockages are detected. There are a lot of questions, a lot of options are given as a patient or a relative, we are concerned as we are taking the right decision or what is the best way to treat.

So the treatment usually offered are either a bypass surgery, which is the open heart surgery whereby the blocks are not removed, but they are bypass through using the normal arteries of the body, which God has given it access. The other way of treating is angioplasty whereby without surgery we go through the groin and we put a balloon and then we put a stent over there and open up the block. The 3rd modality of treatment could be just medical management. So we are always in doubt what is the best way of treatment. Now there are a lot of patients who are in such kind of doubt. This doubt can be removed by a technology called fractional flow reserve. FFR is a technology whereby you put up pressure across the block and you see whether it is hemodynamically significant or not. So the block may be looking severe, but if it is not causing much trouble to the heart then it can be left on medical management.

Similarly, whether an angioplasty is good or bypass is good or just medical management, this FFR technology is the latest technology which helps us and I tried to use in most of my patients wherever these kinds of doubts are obtained. Similarly, there are a lot of patients who are a very high risk that means there are multiple blockages, but they have other comorbidities like they have lungs problem, kidney problem, liver problem and they become very high risk for bypass surgery. Then how do we deal with these kinds of patients? So there are latest technologies which can help us by which we can do a precision angioplasty. So there is a technology called as IVUS, which is intravascular ultrasound or optical coherence tomography, whereby a miniature camera goes inside the heart arteries and we can actually see how the block looks like, whether it is a soft block whether it is a calcified block, what is the size of the block, what is the length, what is the diameter. All these things can be known through these latest technologies and then we can actually decide what are the sizes of the stent that we want to use.

Similarly, suppose there is a lot of calcium. There is one technology called rotablation, which is a diamond-studded burr, which can actually drill the calcium and remove the calcium out of the block and then you can easily do an angioplasty, makes the process very much easier. So these latest technologies, they not only make the angioplasty extremely safe but it becomes very precise and the long-term outcomes also become very good because once you have done the procedure very well, then these patients do very well over a long period of time. Now like for example, in Apollo hospital where I do most of my cases we have all these facilities which are available. So if the patient is on the table and somebody decides that you want to go ahead with a very complex and challenging angioplasty, it is possible to do so at that time and whenever we have these high-risk patients, what we usually do is, we discuss these patients in a heart team.

The heart team at Apollo whereby a lot of doctors we sit together, we decide what is the best way of treating a particular patient. Whether we should do a bypass, whether we should do an angioplasty or whether we should leave it on medical management alone and once we have made a consensus we give it to you. The consensus is based on our personal experience as well as there are scientific scores which are available like SYNTAX score and there are other scores, which tells us that what will be the outcome of our strategy if it is done now or the future and this way the combined clinical approach, using of score and using the latest technology. We can actually make all these procedures which were previously undoable.

So a lot of patients who come to me for second and third opinions, they come with the thought process that a lot of doctors have told that this angioplasty even a bypass is not possible, but the heart is very weak and the patient is suffering. So these are the cases, which are called as CHIP cases (Complex high risk indicated patient), which as a team approach can be done very safely with the use of these advanced technologies. I am Dr. Rahul Gupta, Cardiologist. Today I will talk about valve disease. First, we need to understand what is a valve in the heart. Now the heart is a 4 chamber structure which has got 4 valves. It regulates the blood from one direction to the other. Now blood has to go through the pipes. Now the function of the valves is to open and close properly. There can be 2 kinds of issues like the valve is not opening properly but when it becomes due to the deposits of calcium or some congenital anomaly, so in such cases it may not open and close properly. When it will not close, it will cause leakage and when it will not open, it will cause the obstruction to the blood flow.

Now today I will explain about the aortic valve. When it becomes very narrow, the flow to the different organs will cause the problem. The person can have symptoms of breathlessness, tiredness, fatigue, pain in the chest. There can be fainting, dizziness, collapse because blood is not reaching properly. Till date, the treatment for this kind of problem was to open up the chest and do open heart surgery and replace the old valves with the new valves wither a metallic o tissue valve. Now as the patient desires to get the minimally invasive procedure, so the procedure invented that it can be done without opening the chest. This procedure is known as TAVI/TAVR. This procedure is done through the leg artery. We take the valve into the heart This procedure is done when the patient is conscious. We do not make any kind of any cut. We do the procedure under x-ray guidance.

Now, this procedure is getting done in about 45 minutes. And then the patient is shifted into the ICU for observation for about 24-48 hours, And then the third day, the patient can be discharged. I hope you have understood the procedure through the video. So, this procedure was invented by the great man at the age of 75. When he proposed the idea, people were like that this is an impossible task. But because of his persistent work, we have a wonderful technology which benefitted humanity. I think I have made you cleared enough about this procedure. Similarly, other valves can also be replaced without making any incision.
