Blood mixed with stool is suggestive of inflammatory diarrhea & the source is large bowel (colon). The causes of inflammatory diarrhea are commonly infective (dysentry) and IBD (ulcerative colitis or Crohn's disease). Since you have these symptoms of more than a year, I would request you to meet a gastroenterologist who will order certain blood tests based on your symptoms and a diagnostic sigmoidoscopy or colonoscopy.
