Skin over swelling loose due to heamtoma which gets absorbed later. Sujan or swelling persists as you have probably not taken rest as required during early injury time. Also there might be a coexistent meniscus tear which can be judged clinically and on MRI. You will. Need surgery only when you have instability. Try applying ice packs locally and perform static quadriceps exercises.

Hello, if you have complete ACL injury then surgery is recommended. As your age is also less, it is also an indication for surgery. Torn ACL causes stress on knee joint which may lead to early osteoarthritis. Still if you do not wish to get operated at present, you can try knee brace and hamstring strengthening exercises.

Since you are not facing "instability" issues, you need not opt for a surgery. The loose skin may be due to "wasting"(reduction in circumference) of quadriceps muscle. Swelling can be reduced with proper physiotherapy treatments such as wax bath, IFT and knee rehabilitation exercises.
