Hello doctor ,my father is 76 years of age ,for the last 3 years he is suffering from sciatica pain which increases in the morning during getting off the bed. Hes retired now but sits for a long time he's diabetic and heart patient ,hes tried everything no respite .plz advice.

Dear lybrate-user, We need to know whether it is at all a sciatica. As because presentations of symptoms vary depending on where the primary pathology lies. Management will vary accordingly. For example, if it a musculoskeletal or a neurologic back pain (like sciatica), symptoms are worse with sitting (especially prolonged driving or travel in a flight) and will be relieved on standing for a short period of time or lying supine. On the other hand, pathology at facet joint (a joint of the vertebrae) or osteoarthritis the reverse may be true. Neuropathic pain like sciatica usually has a radicular distribution ie pain radiates along its distribution (back of thigh and legs ). So he will require a good examination and investigations as necessary. We also need to know the status of metabolic factors necessary for strengthening his bony and musculoskeletal framework like calcium, vitamin D etc. Besides knowing his risk factors like diabetes/ osteoporosis etc. So please attend a good Neurologist who can help you out. Consult with your doctor regarding use of vit B12 and drugs for neurogenic pain, physiotherapy and newer analgesics. Control of diabetes is of utmost importance. Check for development of diabetic neuropathy. You can consider MRI if pain is not relieved despite all measures.
