Mostvimportant clue will be obtained from his ECG. You must mention whatvthe ecg report says. This seems to me as episodes of bradycardia and pulsecrate 40 is possible in complete heart block. When such episode happens, you may ask him to fo very light exercises lije moving his arm abovrvthevhead and bring it down. Repeat a fewvtimes and see if his pulse rate increases.in case of complete heart block, there will be no increament of pulse rate with exercise. Waiting on this patient may be risky, so act fast. Take a secong opinion asap, in case of a complete block, a pacemaker is mandatory. Review his medicines he is taking.
