Mild difference in size is normal. If significant difference in size, better to see a general surgeon. They would examine you and if needed ask for an ultrasound. There could be multiple causes of scrotal swelling. Hernia, hydrocele, traumatic hematocele, spermatocele, varicocele and testicular tumour amongst the main causes. If testicular Cancer is suspected, then testicular tumour markers i.e. AFP, B-HCG and LDH would be done. Feel free to contact me directly if you want to discuss this further.
