I was operated on for thrombosis piles in 2005 and have been suffering occasionally with fissure, severe pain, constipation. I am using topical creams (diltiazem/osil/haemamelus etc) and sits bath twice a day. Very cautious in eating. Please suggest remedies for immediate relief as pain is unbearable. If you have painful fissure and not solving with conservative medicinal management its better you check with a surgeon and get operated for the same it might increase the chance of haemorrhoids again.

I think most important aspect is avoiding hard stool, so take plenty of free vegetables, fruits, salads and drink plenty of fluid. Avoid spicy stuff. Next important thing is correct application of the cream. It has to be applied both before and after passage of stool, and it should be inserted inside the anal canal (back passage) though initially it may be painful but slowly you will start feeling better. Take painkiller on sos basis, and most probably you can avoid surgery.
