Dear docs greetings of the day! please guide me, if a patient is on art therapy (bcz of hiv positive) and his cd4 count is 810, viral load isnu detectable, now his doctor want to start isoniazid (which is a antituberculousis drug) along with vitamin b6, just for a prevention/precaution and that too for 6 months. So shall the patient should take this additional medicine? As a pharmacologist what I know that ant tb medicines have antagonistic action on art medicines, also they have lots of side effects in some case renal dysfunction too. Please help me regarding this. Hello, Pl be informed that tb is the commonest oi in HIV positive people. As per recommendations of Naco, india and world health org, ipt is to be given for 6 months, plus pyridoxine, in all HIV pos pts, after of course ruling out "active tb" mandatorily. His treating Dr. must have ruled out active TB for him to start ipt, along with pyridoxine (to prevent peripheral neuropathy by inh). Inh does not cause drug interaction unlike rifampicin and hence ipt is safely recorded. Yes, some base line tests are mandatory before starting ipt.
