There are multiple reasons why you are unable to get better glycemic control. 1. Duration of diabetes: you have been living with diabetes since 19 years and over time, the insulin producing cells may reduce and the ability to produce insulin by the body is minimal and despite using medications, there is treatment failure. Hence, someone like that requires insulin 2. Drug dose optimisation: your dosage needs to be tweaked and then we can check the response 3. Diet/exercise compliance: despite taking medications, if the patient continues to eat high carb diet, medications may not be enough to control the diabetes 4. Drug compliance: poor drug compliance leads to poor glycemic control. Hope this helps.
