I �was 19 when I had a surgery on my right side where there was a tuberculosis man and it was to be removed with a surgery and the infection had reached my right fallopian tube so a part was to be removed to avoid further infection. Now I am 29, I got married in jan 2019. In jan 2020 I conceived but by constant monitoring we were told the fetal heartbeat did not come at all and finally at 9 or 10 weeks I had a medical abortion as it was a missed miscarriage and I did not bleed at all on my own. In july 2020 I was diagnosed with early pcod problem where my dominant egg had to be pushed by use of clomid and hcg trigger then finally after 2 set of continuous cycle treatment in aug 2020 I conceived. My last period was 21st aug. My hcg was 2587 on 1st report and a second report after 2 days showed 4589 (double numbers). At 7w I was shown the fetal heartbeat and the doctor asked me to continue with my folic acid tablet, aspirin 75 mg & progesterone vaginal suppositories for 2 weeks. But after 2 weeks on 10w 1d on a routine check up fetal heart beat stopped and today I should have been 10w 6d. Please can you advice why this is happening again and again and what precautions and tests we should do further to rule out this problem? I will be going in for a d&c on sat 7 or 8th nov as I have again not started bleeding on my own.

As you've got 2 missed abortions consecutively, you should get investigated to know the cause of abortions and accordingly treated. Causes of recurrent abortions are multiple ,but early abortions are usually due to chromosomal anamolies or due to apla syndrome. Or due to progesterone deficiency. Get chromosomal analysis done of fetal tissue during d& c. Get your and your husband's chromosomal analysis done. Get your apla test done. Get your serum progesterone levels done on 24th day of cycle. Get your thyroid profile and blood sugar levels done if have not been done. If abovevtests normal then you can get your tb- pcr of menstrual blood done. Hysteroscopy done to see if endometrial cavity normal.
