Usually surgery followed by adjuvant treatment of chemotherapy or radiation depending on tumor stage is adviced. If tumor is inoperable few patients may begin with chemotherapy to shrink tumor down to make it operable after initial histopath confirmation.

Synovial sarcomas are ideally managed by staging them with help of pet ct or mri to see if complete resection is possible. Based on the age, associated comorbidities, risks involved with surgery and other factors, decision on surgical intervention can be taken. After surgery, if there is disease residue, radiation therapy can be consisered. In case, surgery has risks associated that are life-threatening, we can consider chemotherapy to reduce the size of tumour. This needs to be discussed with a thoracic surgeon and a medical oncologist after thorough assessment of the patient.
