I am taking 90 mg duloxetine to treat facial pain for 7 months. Duloxetine reduced pain intensity from level 6-9, 24/7 to 0-1. Doctor described the my pain as a result of somatic symptoms disorder esp pain disorder. Doctors didn't find any organic causes for my facial pain. Can I taper duloxetine completely? Will tapering bring back pain to initial level? Do I have to take medicine for whole life?

90 mg duloxetine is undoubtedly a high dose. Interventional pain options like sphenopalatine ganglion/ trigeminal ganglion block can be tried to find out what best suits in your case. Once effect of intervention, duloxetine can be slowly tapered off.

90 mg fluoxetine is surely high. I am anesthesiologist intensive care consultant, fipm pain medicine, interventional pain specialist ,cancer pain specialist any pain in my experience will not decrease to 0 by any medice in my experience. Too high dose is never good, kindly decrease it to first 60 mg and observe for 15 days if it's still low at least 2-3 decrease it to 40 mg and observe for 1 month preferably get your complete blood count, liver function test, renal function tests done you can contact your doctor or me.
