My grandmother is injected with 3 anti- vegf injections, there was an improvement after 2nd injection but after few months,the macular edema reoccurred. So,she was given 3rd injection 1 month back but the condition is still deteriorating, there is no improvement. She is on nepafenac, bitabrim,and prednisolone eye drop since 3 months now. How is it that the previous injections restored her vision and she could see a bit but the recent one did nothing? Also, in the 1 and 2 injection given earlier, the doc did not prescribe nepafenac.

Anti Vegf have limited duration of action. Its effect lasts for 2-3 months depending on the salt used in anti Vegf. The effect is not permanent.

Anti vegf injections are given to reduce macular edema and shrink new vessels in retina. Minimum of 3 injections should be taken in 1 month intervals for macular edema. Sometimes there are many factors influencing the result of treatment. First of all I wanted to know what eye disease she was suffering from. After knowing that alone I am able to make comment.

First of all it is important to know the cause of macular edema. You have not mentioned that in you query. Second important factor is type of macular edema and cause of its persistence. Only giving anti vegf injection does not cure macular edema. One needs to find out the cause behind it and that should be targeted. Also nepafenac is not needed after anti vegf injections. So it is okay that your doctor did not prescribe nepafenac after the injection. Also it would be helpful if you share the type of anti vegf injection given and the oct images. Hope this helps.
