I'm facing 2nd trimester miscarriage (1st was pprom17. 5 weeks, 2 and was twins unknown reasons at 20w) 3rd preterm delivery at 28. 1 weeks due to urine infection. That time I had cervical cerclage and taken duvadilan tablet. I'm going to plan pregnancy now, (normal conception) my doc told about heparin injection from conception to delivery can I take duvadilan tablet and heparin injection together? Does I really need heparin injection? What are the indications for taking heparin injection? Is it safe to use daily? Does I need any blood test before starting heparin?

Because you are having second trimester recurrent miscarriages, cause could be cervical incompetence, tendency to have preterm labour, and antiphospholipid antibody syndrome or other hyper coagulable states. Tests are done for all hyper coagulable syndromes like apla and anca antibodies, protein c and s deficiency and if these come out to be positive, then low molecular weight heparin is beneficial. However if cause is cervical incompetence or factors like urine or vaginal infections leading to miscarriage, a cervical stitch and pregnancy support with progesterone will be more helpful. Also you need to keep a watch for any urine or vaginal infections and treat them asap to prevent preterm labor.
