Hello friends, I am Dr Rahul Bhargava, I am a consultant ENT at MGS hospital and have a clinic in Rohini sector 15, Delhi. Today I am going to discuss with you the Sinusitis topic. Sinusitis is a very common condition nowadays and it has been estimated that 20% of the people in India are suffering from Sinusitis and in children around 6 attacks of sinusitis happen in children every year. Sinusitis bahut hi common condition hai, iska ilaj bhi hai magar pehle hame yeh dekhna hai ki kis wajah se hai. Sinusitis kayi karno se hota hai sabse important karan hai ki hamari naak ke andar kuch banawati kharabi ho isme naak ki haddi ka tedapan hona, kuch cells extra hona ya sinus ki opening ke andar rukawat hona. Yeh sabse important karan hota hai Sinusitis ka or iske liye surgery ki zaroorat hoti hai. Toh jin cases mein structure problem hoti hai toh, we need to do surgeries in these cases and remove the problem. The other important cause Sinusitis ka hai woh hai allergy naak ke andar allergic hona, ya allergic rhinitis hona, naak ke jo sinuses hoti hain uski opening ko chota kar deta hai aur usse sinus ki problem hoti hai, kafi cases ke andar pehle patient ko allergy hoti hai aur uske baad wo sinus mein convert hota hai. Toh hum yeh chahte hain ki hum woh allergy ko wahi khatam kar de aur aage woh sinus ki problem mein na badle. Allergy kis wajah se hai, kis karan se hai, kis cheez se hai, wo janana hamare liye kafi avashyak hota hai, jis cheez se allergy hoti hai kuch cheeze aisi hoti hai jisko hum avoid kar sakte hain aur kuch ke liye immunotherapy deni padti hai. Lekin jaise bola jata hai prevention is better than cure, aise hi allergy ka sahi samay pe ilaj se wo aage sinus ki problem mein nahi badalta hai. Ek bahut aajkal common karan hai woh hai pollution, pollution ek irritant ka kaam karta hai sinus ke andar puri nak ki jo lining hoti hai usko usme sujan aa jati hai usse sinus ki problem ho jati hai. Toh uske liye aapko pollution ke area se bachna chahiye, jab jis din jyada pollution hai us din raat ko 9 baje ke baad aur morning mein 6 baje se pehle nahi nikalna chahiye ghar se bahar. Uske alawa mechanical barrier, koi bhi rukawat jo sinus ke raste mein aati hai woh bhi Sinusitis cause karti hai, jisme sabse important cost hota hai adenoid hypertrophy, adenoid ek mask hota hai jo naak ke piche hota hai aur sinus ka jo risha hota hai uske rasthe mein aata hai. Kisi bache mein agar adenoid bada hua hota hai toh kaan ki dikkat ke sath-sath uske nak ke andar sinus ki problem hoti hai. Aur aaj kal sinus ke andar chote baccho ke andar pehle adenoid ki hai surgery ki jati hai, taki woh sinus ki problem se nijat pa sake. Uske alawa jo sharir ki immunity hai wo bhi ek bahut important cause hai ki hamara sinus ki problem hone ka, immunity mein sabse jyada aaj kal Vitamin D ka role aa raha hai. Jin marijoo mein Vitamin D ki kami hoti hai unke under sinus ki problem bhi baar-baar hoti hai. Toh sinus ka ilaj jo hai woh sambhav hai kuch patients mein hamme surgery karni hoti hai, jiske andar koi banawati kharabi hui koi cell extra hai, jo ki hamme CT Scan se pata lagta hai usme hamme surgery ki zaroorat hoti hai. Jin baccho mein adenoid bada hua hai, hamme adenoid ki surgery karni padti hai, agar allergy ki problem hai toh allergy ka ilaj karna padta hai, toh isse tabhi hum is problems se completely nijat pa sakte hain. Today through this medium, I am going to talk about a very common problem in children and adults that is an earache or the pain in the ear. The pain in the ear can be due to local causes in the ear or it can be due to referred causes where the problem lies somewhere else and it is presented as pain in the ear. The common causes of an earache is especially in children is Otitis Media with effusion, which is water into the ear, otitis effusion which is the infection of the external part of the ear, Acute Suppurative Otitis Media which is the formation of pus behind the tympanic memory, a forum body or impacted earwax, these are few common problems which can lead to an earache and can be very troublesome in children. A forum body needs to remove as soon as it is detected, if you have impacted earwax and it is causing a problem then it needs to go. If you have acute suppurative otitis media there which is the inflammation and the pus behind tympanic memory then the usual cause lies in the nose. So we have to treat both the nose and the ear to solve the problem and prevent such frequent episode in the later on. Otitis media effusion is the very important cause of an earache in children it also leads to hearing difficulty and leads to developmental problems in children. The adenoid hypertrophy is a very usual cause which leads to otitis media with effusion and we need to tackle also along with an earache. The other important causes are the referred causes where the problem lies either in the temporomandibular joint which is the joint just in front of the ear or in the throat any infection in the throat or any problem in the throat can give rise to pain in the ear.
