Hello,

I am Dr. Neha Jain, consulting gynaecologist and obstetrician and infertility specialist. Today I am going to talk about Antenatal health check up. Jaise ki hum jaante hai ki pregnancy mein mother and child dono ko hi attention ki jarurat hai, aise mein konsi dawaiya khani hai, konse investigations karane hai iske baare mein mei apko aaj bataungi. Pregnancy ko hum three trimesters mein divide karte hai. Shuru ke three months, middle three months aur last ke three months. Shuru ke three months mein baby form hota hai isiliye hum aise time mein baby ke liye vitamins folic acids advice karte hai. Kyunki Indian population mein ek aisi enzyme jo folic acid ko uske active form mein convert karti hai vo kam payi jaati hai, toh isiliye uska active form methylfolate aise mein better rahta hai. Folic acid ke sath sath hum aur bhi vital vitamin bhi dete hai jaise ki DHA, methylcobalamin etc. Iss time hum ultrasound bhi aam taur pe karwana pasand karte hai kyuki ultrasound se baby ki correct dates ke baare mein hume pata chalta hai. Agar humare paas mein first trimester mein ultrasound hota hai toh baby ki final date of delivery hum ek confirmed roop mein bata sakte hai. Ultrasound karane ke baad hum mother ko kuch blood tests advice karte hai jaise ki haemoglobin, thyroids, blood sugar then STDs ke testing like HIV, Hepatitis B, Hepatitis C, syphilis ka testing, urine routine microscopy ye advice kiya jaata hai. Fir uske baad ka phase aata hai 11 to 12 weeks pe hum aam taur pe aajkal ek ultrasound karate hai, level one ka ultrasound. Iss ultrasound mein baby ki ye waali bone aur ye wali bone ko dekha jaata hai, ki uski kya thickness hai. Kyuki kuch babies mein ek anumanshik beemari down syndrome naam ki payi jaati hai toh ye iska screening test hai. Aur level one ultrasound ke saath saath hum double marker ka blood test bhi karwate hai, jis se ki 90% tak hum iss beemari ko rule out kar sakte hai.

Fir second trimester ki baat aayi. Second trimester mein iron and calcium diya jaata hai agar mother ka haemoglobin already bahut achcha hai, above 12 hai toh aise mein iron avoid kiya ja sakta hai. Fir usi time pe level two ultrasound 18 weeks pe advice karte hai. 18 to 20 weeks pe level two ka ultrasound advice kiya jata hai, jisme baby ki sari body facts dekhi jaati hai from head to toe. Isme baby ka heart, lever, kidneys, lungs sabka hum analysis bhi karte hai. Iske baad aata hai mostly iron calcium continue kara jata hai aur mother ka blood sugar ka screening kara jata hai, usse glucose pilake uske do ghante baad blood glucose ka level check kiya jaata hai sath mein haemoglobin aur urine routine toh har trimester mein check kiya hi jata hai. Then 32 weeks pe bhi ajkal doctors ek ultrasound karna pasand karte hai, jisme hum baby ke growth ke bare mein jaan sakte hai, baby ke aas paas kitna pani hai ye jaan sakte hai aur uska avalnal upar hai, neeche hai iske vishay mein pata chalta hai. Aur fir finally aata hai term ultrasound jisme hum baby ki position jante hai, baby ke aaspaas kitna pani hai ye jante hai, baby ka weight kitna hai uske baare mein pata chalta hai aur uss hisab se hum normal delivey ya caesarian ka decision le sakte hai. Toh ye thi three trimesters ki apke liye description.

I am Dr. Neha Jain, Gynaecologist. Today I will talk about women's health scenarios in our country and cancer in women. We all know that women's health is neglected in our country. Whether it is a health of working women or a homemaker. They are doing multitasking to such an extent that they are compromising on their health. What is health? Health is a state of physical, mental wellbeing. So, today I will bring about some current statistics regarding women health in our country which may be an eye-opener for all of you. World economic forum ke hisab se humari country gender equality mein worst ata hai. Our rank is 132 out of 187 in terms of gender inequality. Gender inequality kaise calculate ki jaati hai. Iske parameters physical, metal and social health se hi related hain. Usmein hum kafi poorly perform kar rahein hai.

Agar hum other statistics mein dekhein toh female doctor ka percentage dekha jaaye toh it is just 6%. Which means 20,000 population mein 1 female doctor mil rahi hai. Generally India mein females, female doctor se hi milna pasand karti hain. Male doctor ko dikhane mein ek tabboo mana jaata hai. Aise mein aap soch sakte hain ki kya access hai health ki taraf. 50% of ladies mein anemia paya jata hai and 60% women mein anemia hai. Iska common cause hai iron deficiency. Kyuki proper nutrition, health education nahi hai. Shayad unke family members bhi kahin na kahin avoid kar rahein hain. 40% women Indian mein HIV/AIDS ke cases represent kar rahin hain. Aam toor pe ye women mein common dekha ja raha hai. Aam toor pe gharon mein yahi kaha jata hai ki men gharon mein jyada kam karte hain. Apne emotions ko heart mein rakhte hain islia unmein heart health risk jyada rehta hai. But that is not the truth. 60% health issues ka burden hum apni country mein carry kar rahein hain. Jismein cardiovascular health issues ki vajaha se women mein death jyada common hai as compare to men.

Next topic mai cover karungi cancer in women. Breast cancer abhi women mein jo 2nd number pe ata tha, ab India ka top most cancer hai. Breast cancer rural population mein kam hota hai. 1/22 urban ladies min breast cancer paya jaata hai. 1/60 rural area mein ye problem dekhi jaati hai. Self-examination best way hai breast cancer ko diagnose karne ke lia. Vo lump check kar sakti hain and fir doctor ko contact kar ke further screening and diagnose karwa sakti hain. Breast cancer unmein jyada common hain jinmein family history dekhi jaati hai. Ismein marginal risk tab bhi badhte hain agar woman 30 years ke baad conceive karti hain ya fir alcohol ka intake ek glass se jayada paya jaata hai ek din mein ya fir jinhone baby kabhi conceive hi nahi kia hai.

Now the 2nd most common cancer in women is cancer cervix. Ye uterus ke mouth ka cancer hai. Ye rural population mein jyada common hai. Iska common cause human papilloma virus hota hai. Iski rate and death jyada increase ho sakti hai. Agar ladies tobacco ya related products consume karti hon, genital infection ho and HIV hon toh bhi death rate jyada badh jata hai. Cancer cervix ke lia screening test uplabadh hai, usko hum pap smear kehte hain. Ye test har lady ko 3 years mein ek baar karana chaiye upto 65 years of age. Is se hum cervix cancer ko early age pe catch kar sakte hain. It is preventable cancer. Agar aap ko koi bhi issue lagta hai toh aap apne doctor se contact kariye. Aap apni health and aas pass ki ladies ki health ka dhayan rakhieye.

I am Dr. Neha Jain. I am a consultant gynecologist and obstetrician and also infertility specialist.

I have done my MBBS and post graduation from Lady Hardinge Medical College, currently, I am running my own clinic with the name Dr. Jain's bliss and bless women's health clinic and Dr. Jains Pathlab in Mehrauli. I am also associated with a visiting consultant with Hospitals like Fortis Hospital and Adiva Hospital.

Today I would like to talk about PCOD that is Polycystic ovarian disease as such the incidence of PCOD reported worldwide is something around 7 to 22% but in our day to day, a clinical practice we see one out of every 2 patient is suffering from diseases. Now first I would like to talk about the symptoms of PCOS, the commonest symptom of PCOS is menstrual irregularities that are there will be daily cycles, short cycles or frequent cycle these all are signs of anovulation. The next symptoms are the signs and symptoms of hyperandrogenism, androgen is like male hormones and here we will see that the patient may have hirsutism that is excessive hair growth or male pattern baldness or acne. If you go to your doctor then your doctor will suggest you some test and on these tests, you may get reports like access free testosterone levels or free androgen index is high. Then the third symptoms is of polycystic ovaries on ultrasound, if one is having polycystic ovaries on Ultrasound that does not mean that the patient is suffering from Polycystic Ovarian Syndrome, in fact in people with polycystic ovaries only about 20% have problem of PCOD, then your doctor with rule out other causes of hyperandrogenism that is hypothyroidism or hyperprolactinemia. Now PCOD is associated with mostly with obesity and the lifestyle issues, it is also seen in ladies with a family history of diabetes or metabolic syndromes. Now the foremost important thing is I would like to talk about is the treatment, of course, lifestyle modification is the first thing which we advise to our patients. In lifestyle changes patient need to do exercises and almost 30-40 minute exercises per day are recommended which have to be moderate-to-severe in nature. Then dietary changes need to be made, patients are advised to avoid fried food, fatty food, junk food than sweets, sugar, empty calories have to be avoided. Reduction in 5% of the weight can result in 50% improvement of your symptoms or chances of women to become pregnant. So that is the mainstay of treatment then next your doctor will suggest you some medication like metformin or combined oral contraceptive pills.

So, in short, that is what I wanted to talk about PCOD which is mainly a lifestyle problem and you need to improve your lifestyle in order to get rid of this disease.
