Body ki shape maintain karne ke lia bhi hum surgical procedure karte hain. Face pe sabse jyada hair transplantation kia jata hai but agar ise alag subject rakhein toh rhinoplasty hai. Iske 2 components hain. Ek ENT surgeons karte hain jismein septum involve hota hai. 2nd hai aesthetic rhinoplasty. Ismein septum and puri nose mein agar koi kharabi ya tedha pan hai toh use thik kia jata hai. Rhinoplasty ki market mein humesha need rahi hai and log iske lia aate hain. Par iske sath important chiz hai ki rhinoplasty mein kuch log marriage ke pass aate hain ya jaw public relation mein hain. Dusra rhinoplasty ke lia vo aate hain jo disturb hain body image ko leke. Body image ke lia jo aaye hain unhein counseling ki jarurat hai aur btana jaruri hai ki is se hum kya aur kitna change kar payenge. Nose mein bone ke alaw cartilage hota hai and iski apni memory hoti hai. Toh aap use kitna bhi sidha kar do par vo 10-20% vapas apne natural look mein jata hi hai. Ye chiz patient ko baad mein disturb karti hai. Agar kuch patients photo leke aate hain ki humein aisa banna hai toh aap ko careful rehna hai. Kyuki agar aap ko lagta hai ki surgical same result possible nahi hai toh aap mana kar dein. Kuch patients suicide attempt tak karte hain.

Aise patients mein psychiatric evaluation bahut important hai. Uske baad use realistic approach samjh aa jaye on paper dikha ke tabhi hi surgery karni chaiye. Rhinoplasty kuch congenital cases mein hoti hai jaise cleft issue hai ya nose mein tedha part hai. Septum aap ka company insurance mein cover hota hai but aesthetic aap ka insurance mein cover nahi hota. Rhinoplasty 16-18 saal ke baad hi aap ko di jaati hai. Nahi toh ye growth mein interfere karta hai. Islia age factor ka dhyan rakehin. Agar kisi trauma ya fir infection se nose ki shape mein isue aaya hai toh unko explain kar ke hum treatment dete hain. Rhinoplasty ka treatment number mein increase kar gaya hai. Image se bhi patients ko effect padta hai. Maan lijiye ki 2 OT mein se 1 toh rhinoplasty case hota hi hoga. Sabse common hai ye. Next common hai lip se related, eyelid se related ya fir face pe aur koi problem. But mai rhinoplasty ke baare mein hi baat karunga. Rhinoplasty closed and open pe bahut debate raha. But ismein major fark nahi hai. Ismein naak mein niche ke side chota sa cut ata hai, usi se open ho jata hai and koi major scar bhi nahi aata hai. 90% rhinoplasty open hoti hai puri duniya mein. Open mein time lagta hai 3-4 hours ka Puri tarah se 2-6 mahine lagte hain sujan jaane mein. Par aap 3 weeks mein normal life mein aa sakte hain. Iske alwa agar aap ko koi information chaiye toh aap mujhse consult kar sakte hain.

Male breast enlargement pichle 10 saal mein dekhein toh bahut tezi se badh raha hai. Har 1 OT mein ek gynecomastia ka case milta hai. Obesity se related gynecomastia alag hai. Par routine care karne ke baad bhi gynecomastia ka problem hai. Obesity mein agar aapne weight loss kia hai tih aap ke breast bhi hang kar jaate hain. Liposuction karne ke baad bhi dekha jata hai ki breast aur jyada visible hai. Bachon mein ye problem kafi common hai. JUnk food khane se bhi ye problem aati hai. Bache humare pass 13-14 years mein hi parents ke sath aana shuru kar dete hain.

Unko class mein bahut embarass feel hota hai. Kya humein ye chiz karni chaiye? Male breast enlargement mein kya hota hai? Ismein humare 2 components important hain. Breast ke glands males mein bhi hote hain. Vo increase karne lag jaate hain. Body building karne ke lia jo hum supplements lete hain, unmein hormones hote hain, uski vajaha se gland badh jata hai aur fat bhi badh jata hai. Aap kitna bhi gym kari ya exercise kar, ye chiz badhi hi rehti hai. Aise mein humein surgical treatment hi dena hota hai. Ye ek scarless treatment hai kyuki pin hole ke through hi liposuction kia jata hai fir gland excise kia jata hai. Treatment ke baad thode se precautions hote hain 1 month ke lia.

Apki elbow over the shoulder nahi jaani chaiye, heavyweight nahi uthana hai. Lower body mein aap weight utha sakte hain. Same day se ya fir next day se aap driving bhi kar sakte hain. Aap ki routine life mein koi farak nahi padta hai. Kewal jahan aap ko weight utha kar chalna hai vo 3 weeks ke lia mana hoti hai. Swimming 3 weeks ke lia aap ko mana ki jaati hai. Baki chizen aap 3 weeks onwards resume kar sakte hain. Ye kafi safe surgery hai. Aur weight loss ya fir bariatric surgery ke baad jo weight loss hota hai, usmein incision ki ek line jarur aati hai. Kyuki body ki almost sari chizen lift karni hoti hain, islia ye bhi lift mein aata hai. Jahan bachon ki baat hai, jahan tak avoid kar sakte hain like vo jab tak 18 ke nahi ho jaate, hum surgical intervention avoid karte hain. Par kuch bache psychologically jab bahut jyada depress ho jaate hain toh humein touch karna padta hai.

Dekha jaata hai ki bache school jaane ke lia ready nahi hote hai, parents bhi panic hoke aate hain, So, parents ke panic hone se koi surgical intervention nahi hai. Par bacha agar school hi nahi ja raha hai and kisi activity mein participate nahi kar raha hai usmein fir aap bata ke written concent ke sath kar sakte hain. But ye ideal nahi hai. Ideal 16-18 saal ki age ke baad hi hai. Aap ko pressuer garment pehnna hota hai, jacket type ka. Thi se ki gayi surgery ko dubara nahi karna padta. Kabhi kabhi ek breast badhta hai dusre ke mukabale. Single breast operate kar ke aap long term result nahi le sakte. Islia dono hi operate karne hote hain. Injection deke jo animal produce kia jaate hain ya fir unhein bada kia jata hai, unka intake harmful hai humare lia.

I am Dr. Lalit Choudhary, senior consultant, Department of plastic and cosmetic surgery. Today I will discuss about the one important cosmetic surgical part which is body contouring. Body contouring comprises of almost 50% of the plastic cosmetic surgery procedures and if you count contouring can be anything from head to toe but most commonly the contouring if you see in the face, you can contour the wholeness of the face, you can fill with fat graft or you can do genioplasty or rhinoplasty with the increase or decrease in the size with the many fat grafts and fillers available in the market. We as a plastic surgeon prefer to have the autogenous tissue in which is fat, then we have breast augmentation and reduction procedures which is a subject in itself and we nowadays many patients are coming up with fat injection for increasing the size of the breast rather than you going for the implants. You may require 2-3 sittings but in the end, there is no cutting, there is only in aspiration of the fat within syringe and injection of the fat with injection.

Nobody in the future can make out if you ever had any surgical procedure, nobody. So that is important mostly for the patients like of the unmarried age or who are getting married again or patients who are afraid of foreign material like silicone in the breast. So that is an important point. Then we have abdominoplasty and liposuction. Liposuction has many myths like when you do liposuction then it comes back which is commonly seen in every other patient is asking about this question. So, it is important that there is nothing like that in liposuction. You take out a number of fat cells and the residual remaining fat cells will never ever multiply in number. It is not a tumor. These areas from where we do the liposuction those areas will have now only 30 to 40% of the cells left, 60-70% will be taken out, that is how the liposuction works. It is not a weight loss procedure. It is important that it's only a body contouring procedure. You lose some weight because you take out something which is secondary.

Then after a limit if you have a overweight status like in 120-150, then obviously you need a bariatric surgery and not a liposuction or a tummy tuck. Then the next is the tummy tuck. Tummy tuck is very important. It is not just gonna help you in contouring, it also helps in future because of change of the centre of gravity with the hanging apron which is taken care of. You have back pain, knee pain and other pains in the midline because of the tummy which is bloated and hanging down. So that is taken care of with a tummy tuck and then there is muscle plication done in the midline that gives strength to the midline of the abdomen muscle, a cross set of the abdomen. So, this is very important and it is one aspect so patient who are obese but not qualifying for bariatric surgery can have tummy tuck. Then patients who are not obese they can only for the cosmetic reason come, this is not the medical advantage of loss of girth and leading to advantage of back pain and the knee issues taken care of. Only cosmetics if you see that after 2 or 3 deliveries that is hanging lower abdomen commonly seen. More and more patients are coming after the second delivery for the tummy tuck.

Patients are asking for simultaneous tummy tuck during the cesarean caesars when they are getting it done, so it's very common now. So, these are the routine contouring procedures. They can be done in obese as well as after the bariatric surgery. After bariatric surgery you do contouring of head to toe of almost everything in the body, everything hangs down but they are the patients who need the bariatric surgery for the metabolic advantage. They are often associated with thyroid and blood pressure and diabetic issues. Many of them require less those or get corrected with the bariatric surgery. So, it is not that you can get away with the liposuction or tummy tuck by not and you can avoid a bariatric procedure so there is no fight. They are very defined zones in which we have to do either.

In cosmetic surgery I would like to tell you- one thing is very important is that patient must be explained the possible results and that the outcome which a surgeon feels is 100% but the patient may not feel same. So they are dissatisfied. The realistic expectation of the patient must be told to the patient and kept in mind while you are taking the patient to the surgery. But keep in mind that every patient must be explained the possible outcomes. There are few contraindications and there are few complications associated with all the procedures which must be told to the patient. They should not be very satisfied with even the 100% result. So we do other possible associated plastic surgery subunits also like reconstructing, like on core surgery, trauma reconstruction.

We have all range of surgeries - not just reconstructive plastic surgery but also the cosmetic surgeries with us. Nowadays , gradually the cosmetic surgery cases have gone up by more than 50% of the plastic surgery practices. More common cosmetics side patients who are coming to us, are about body contouring in both the males and females.

In males, we see nowadays very common , is the male breast tissue with enlarged breast or what we call as Gynaecomastia. It can be completely cured with a small operation which is practically scar less. Generally next day , you can join with your routine work and there is 3 weeks of weight bearing issue, otherwise table job and walking is fine.

Same in females, there is tummy tuck, breast enhancement and reduction. Most commonly for face, we come across patients coming for rhinoplasty or the nose job.  Then the hair transplant procedure where big number of patients suffering from baldness or loss of hair come.

Generally other than the cosmetic procedures, we have well-developed plastic surgery department set up at our Ganga Ram Hospital. We also practice as a clinic basis in Dwarka where we generally see the patient. There is no operation facility there. That is basically the post-op care of the pre-op care for the patient.
