Emergency Contraception
Emergency Contraception

Hello!

I am Dr. Indu Taneja, Head of the Department of Gynae and Obstetrics. Today I will talk about emergency contraception. Emergency contraception is when a contraception is taken after an unprotected sex act. This is very commonly used nowadays. People think that as a method of contraception but this should not be used as a normal method of contraception. This is only for emergency and should not be made as a routine. So there are various drugs which can be taken as emergency contraception. First, one is called pill 72. You must have all heard about this. This means a pill which is taken within 72 hours of the unprotected sex.

This is a progesterone pill. The dose is approximate 1.5 mg of progesterone. It is taken within 72 hours. Then the second drug which is more effective is - Ulipristal. 30 mg tablet which is now available in the India, can be taken till 5 days of the unprotected sex. This is more effective as compared to the progesterone pills. Then the third one is- combined pills which has both estrogen and progesterone. This can again be taken within 5 days of the unprotected sex. This is also equally very effective but the most effective is the Ulipristal. Then there is the contraceptive device IUCD. It is a kind of Copper-T. This can also be used till 5 days after the unprotected sex. This is quite effective. In any case fertilization has happened, this prevents pregnancy to grow inside the uterus. In case somebody has taken these pills and after that she wants to conceive then these medicines are safe, specially the progesterone pills. This is quite safe, even the combined pills are safe. So no side effect are reported in the baby after the emergency contraception. But please please do not use them as regular method of contraception because all these medicine can lead to irregular periods, delayed periods. So this can only be used once in a while not regular.

Thank You!

Abnormal Uterine Bleeding
Abnormal Uterine Bleeding

Hi,

I am Dr. Indu Taneja, senior consultant and head of the department gynae and obstetrician, Fortis Escorts Hospital, Faridabad. Today I will talk about abnormal uterine bleeding. The normal cycle is for 3-5 days and cycle recurs after 25-35 days. If the bleeding is more than 5-6 days or if the periods happens before 20 days or happening late after 35 days, it is considered as abnormal. In a young age, it is very common. Most likely because of hormonal issues or PCOD. Abnormality in the bleeding at middle age or reproductive age can be because of abnormality in the pregnancy or abortions. Today I will talk about abnormality in the bleeding in pre-menopausal age.

So, it should not be taken lightly. If bleeding is heavy or frequent, do not take it lightly and consult a doctor immediately. 40 ki age me periods me abnormality hoti hai to logon me myth hai ki periods band hone vale hain islia periods me disturbance hai. But ye normal nhi hai. Apko iske lia doctor se hi consult krna jruri hai. Jab periods band hone vale hote hain to periods delayed hote hain. 2-3 months ke gap me aate hain. Or fir band ho jate hain. Agar period 1 saal tak band rehte hain to use hum menopause kehte hain. Agar apke periods jldi aa rhen hain ya jyada aa rhen hain to this should not be taken lightly. Immediately doctor ke pas jana chaiye. Sabse important test isme ultrasound hota hai.

Is se hume pta chal jata hai ki hormones ka problem hai ya koi massa ho rha hai ya fir tumour or fibroid hai. Jo bhi cause hota hai, uska treatment is available which can be easily done through laparoscopy surgery. Hysteroscopy ke through isko treat kia jata hai. Isme rest ya precautions ki jrurat nhi hoti hai and patient bilkul thik ho jata hai. Islia 40 years ki age ke baad koi bhi abnormal bleeding hai to ise lightly na len or doctor ko dikhayein and treatment len.

Thank You!

PCOS (Polycystic Ovarian Syndrome) - Causes & Treatment
PCOS (Polycystic Ovarian Syndrome) - Causes & Treatment

Good morning! I am Dr. Indu Taneja working as Head of the Department of Gynae and Obstetrics in Fortis Escorts Hospital, Faridabad.

Today I will be talking about PCOS commonly known as Polycystic Ovarian Syndrome. This disease affects all age group women and mainly the young girls. Then it can affect the females of childbearing age and those of after the childbearing age also. The commonly affected females are the young girls.

This is caused mainly because of hormonal imbalance. This hormonal imbalance leads to the symptoms which are mainly irregular periods. Mostly, patient have delayed periods such that even women may not have periods for long months altogether. Then there is excessive weight gain. It can also lead to excessive abnormal hair growth on face and the chin and acne. On the top of it , there is hair loss from the scalp also. So this is all because of the hormonal imbalance. This hormonal imbalance later on can lead to infertility.

So the treatment is basically the change in lifestyle. This disease is mainly because lifestyle is changing nowadays. There is more of sitting work. The girls are not doing any extra activity. Most of the times, they are sitting and not doing any kind of exercise or exertion. They are just working on the laptop or maybe just studying and eating a lot of junk food or consuming a lot of cold drinks. This leads to excessive weight gain. This weight gain in turn leads to hormonal imbalance. So it becomes a vicious cycle.

The first and foremost important treatment in this becomes the change in lifestyle. Start doing any kind of exercise. Any kind of workout for at least 25 to 30 minutes. Cut short on junk food. No cold drinks, no aerated drinks. No dabba band things to be eaten. Exercise is the foremost important thing. Once they start reducing weight, automatically the hormonal imbalance changes and then they start responding. And after the lifestyle change, there is the symptomatic treatment. Treatment is to be given for irregular periods- maybe hormonal pills. Then if there is insulin imbalance then the patient can be given symptomatic treatment for that also. For excessive hair growth also. But to reduce this, to come back this, the most important is the change in lifestyle. I think for the young girls, the take-home message is that they should do some kind of exercise.

Laparoscopic Surgery - An Insight!
Laparoscopic Surgery - An Insight!

Good Morning! I am Dr. Indu Taneja working as head of the department as gynae and obstetric depatment in Fortis Escorts Hospital, Faridabad.

Aaj hum aapko laparoscopy ke bare mein batana chahenge. Laparoscopy - durbin dwara surgery ya jisko aap log laser surgery bhi bolte hain. Ye laser nahi hai balki durbin dwara surgery ki jati hai. Ye aaj kal latest technique hai.

1-2 din me aap major operation ke baad ghar ja skte hain. Blood loss bahut kam hota hai. Aapki recovery bahut fast hoti hai. Aap apna kaam bhi jaldi se jaldi start kar sakte hain. Bed rest ki zarurat hi nahi padti hai. Immediately aap apni routine activity start kar sakte hain. Cosmetically bhi ye surgery bahut achi hai, quki aapke peat pe koi cut nahi aa raha hai. 1-2 chote cut hote hain. Jo baad mein dikhayi nahi deta hai. So, ye saare advantages hain laparoscopic surgery ke.

Par jis chiz ke advantage hai, uske sath disadvantages bhi hote hain. Disadvantage ye hai ki surgery thodi expensive hai quki iske saare instuments expensive aate hain, unki maintainence cost kaafi zyaada hoti hai. Plus isko karne ke liye experts ki zarurat hoti hai because uske liye apko training di jaati hai. To ye thode se drawback hain.

Otherwise, apka koi bhi operation durbin se hota hai , to apke liye sabse jyada fayedemand hain. Or hamare field me, gynae me aaj kal har parkar ki surgery durbin se ki ja sakti hai. Most common jo females me ki jati hai vo uterus removal hai. Jisme aap 2-3 din me hi ghar ja skte hain or 1 hafte me apna pura routine kitchen work kar skte hain.

Or bhi jo gynae ki surgeries hain, ectopic pregnancy hai, ovarian cyst hai, endometriosis hai, koi bhi even cancer surgeries bhi hain, ye sab aap laparoscopy se krva skte hain. Laparoscopy sabse bda boon hai. Jiska jyada se jyada advantage uthana chaiye. Jiske bare me jyada jankari hogi to aap iska fayda utha payenge. Thank You!

Tips about Menstrual cycle, Safe periods and various methods of contraception.

I am Dr. Indu Taneja working as a senior consultant in Fortis Escort Hospital Faridabad. My special interests are high risk obstetrics and laparoscopic surgery. Today I will talk about contraception little about the normal menstrual cycle.

A normal cycle is of approximately 28 days so the ovulation means the egg formation occurs in the middle of the cycle that is approximately 14 days but this day is not fixed it can be one or two days prior or one or two days later. So let’s say if it happens on 14th day four or five days before the egg formation and four or five days after the egg formation is the period which is the danger zone that means the pregnancy can happen in this period if you have intercourse during this period so this is the danger zone. So if one happens to have the relation in this period one can use emergency contraception which is you must be all aware of pill 72 or unwanted pill which is to be taken 72 hours within this period the pregnancy is to be avoided.

Next will be other contraception like the barrier method which is normally condom. This is not the foolproof method. This has the maximum failure rate approximately 35 percent. This is mainly effective in the period when the patient is breastfeeding. At this period this is the common effective method. Then there are the other methods you have heard about the pills and the Copper T. The pills are the second commonest method in which every day one has to take the pill for 21 days. The main drawback of the pill is you have to remember you have to take it on a fixed time and fixed day. You should not miss even a single dose. If one misses a dose you can have breakthrough bleeding and the failure also happens. So this is the main drawback of the pills but otherwise the safety profile is very good.

Other method is the IUCD that is the Copper T which is inserted inside the uterus. This has again the same efficacy as the pills but the advantage over the pills is you are not supposed to remember this. This once inserted can be kept and can be used for three years to five years. Government has also provided the intra uterine device which is effective for five years it is as effective as the contraceptive pills but little drawback of this is sometime this can cause irregular bleeding or the heavy bleeding during the periods which can be taken care of giving some kind of medications.

If you need any further clarification or any kind of clarification or consultation you can consult me on lybrate.

PCOD
PCOD

Hello, everyone. I am Dr Indu Taneja, working as a Senior consultant at Fortis Escorts Hospital, Faridabad. My area of interest is high-risk obstetrics and laparoscopic surgery. Today I will speak about PCOD i.e, Polycystic Ovarian Diseases.

This is a disease which is affecting a maximum number of girls these days. In my OPD, there are 10-20 % of the girls are suffering from PCOD. The main symptoms of this disease are;

The most common cause of this disease is the change in lifestyle of the young generation. Nowadays, the young girls are just sitting, chatting on their phone, keep sitting in front of the computers, having a lot of junk food like old drinks and hardly go for any outdoor activity. All these leads to increase in the weight gain and hormonal distribution changes which are hormonal imbalance leading to the symptoms of irregular periods and hair growth.

Treatment-

This disease is treatable. This disease can be treated very easily by your own will power.

  • You have to change your lifestyle. Changing lifestyle means doing any kind of exercise like yoga, gym, atleast 4-5 days in a week.
  • Cut down your junk food.
  • Do some kind of dieting. This will lead to the reduction in your weight. If the weight is less than definitely, the hormonal imbalance is going to be better.
  • Of course, the medicines are required to treat this disease but medicines are not as important as a change in the lifestyle. The medicines can be required for a long time also. But if you continue losing weight then the medicines will be more effective.

If you want to consult me the treatment or for any other problem, you can connect with me via. Lybrate for the video call or audio consultation. Also, you can come to my clinic which is Fortis Escorts Hospital in Faridabad.

Thank you.
