Bicalutamide can be given only for a month or so but as effect is good than continue but this is not standard treatment I would advise hormone therapy with leuprolide or zoladex irbil orchidectomy.

Your father has metastatic ca prostate.
He requires hormonal therapy first- medical or surgical.
Medical therapy includes 1 injection every 3 to 6 months and caluran 50 mg along with it. Surgical means castration (removing both testis by surgery).
Depending on his response to hormonal therapy he can be planned for further management like chemo, abiraterone etc.

The question is not very clear. If you have a metastatic ca prostate and you were put on hormone ablation therapy your disease should be regularly monitored with your psa values.

Dear Mr. lybrate-user,
what treatment you father has taken for cancer prostate. Radiotherapy? If yes then he may have burning in urine. If not he might be having urinary infection/ other causes and should be investigated with ultrasound (kub) with post void residual urine, urine routine and culture sensitivity.
Once checked treatment can be suggested.

My dad is dealing with prostate cancer and metastatic i.e. Spread to bones. He is heart and diabetic patient as well. He had undergone surgery to remove testicles almost year back. Psa test shown low reading - 0.140 after surgery but start increasing gradually since then. Now psa stands at 18 and just had 2nd pet scan. Doctor compared both pet scans (6 months gap) before and after surgery and did not alarm anything new. No bone pain as such currently. Based on doctor prescription he stated zoledronic acid injection on monthly basis. 2 injections so far. He currently takes bicalutamide (50 mg) along with other calcium and vitamin d supplement here are the 2 major issues I need help1. After zoledronic acid injection. He got light fever for a week and nausea. His psa score was very low after surgery but gradually increasing in recent months now 18. He is taking bicalutamide (50 mg) (1 tablet daily) since november 2022. What are treatment options to treat the rising psa. I can share the pet scan and other reports. I am looking for a senior doctor opinion and follow up appointment. No homeopathic doctors please.

Cancer staging needs to do first. Different treatments / approaches are available for different stages. Psa will stop increasing only if we are able to treat the cancer or stop the cancer growth. Zoledronic acid & bicalutamide have limited role. Pet scans. Other reports & most imp patient s clinical condition will decide further treatment.
