This is Dr. Deepam Shah, Today we are going to discuss about the very much talked about treatment of anti-aging. Kam age, kam fine lines, kam wrinkles, kam skin sagging everyone doesn't like the look of skin sagging and so what are the treatments which we have for anti-aging. So, the first thing is prevention. So how do we prevent fine lines, anti-aging and sagging? So, the first thing which you need to do is have a good, wear a good sunblock because sunscreen is a very important part of aging. The more the sun falls on your face, more you age faster because the collagen gets degenerated and you age faster. So, we will start with the anti-aging treatments into three parts: one is which is non-invasive treatments like cream-based treatment, second one is the minor invasive treatment and third one is a surgical option.
So, first one which we are going to talk about is the non-invasive cream based. So, there are creams which are available in the form of retinoids. They are basically, they modify the collagen and stimulate new collagenesis. Second one is peptide-based things. What the peptide does it goes into the skin and it stimulates the fibroblasts and leads to collagen regeneration. So that’s the second one. Now, we come into a semi invasive one which is there like Botox and fillers. So, Botox is basically some kind of injections.
What it does? It goes into our muscles; it paralyses the muscle so that the muscle action doesn't happen and you get a tighter look on your face. Fillers is the one of the better treatments, one of the best treatments for anti-aging which is there because it gives you an instant volume. As you age the volume of your face tends to go down. The filler immediately replaces that with the fat and you have a smoother and a tighter skin immediately. One of the other treatments in the semi invasive one is the laser resurfacing.
So, there is a fractional CO2 laser resurfacing or micro-needling RF which it does it basically remove the surface blemishes and the surface textural changes and also leads to collagen regeneration which makes your skin smoother and fine lines and wrinkles improve much better with that. The last and the final one is the surgical option which is called as the cosmetic surgery. I am Dr. Deepam Shah, Dermatologist. We will discuss today acne scars. This is a very common problem because of the acne scar which is preventable in early life you treat it at the right age. So, I want to discuss how should we go in treating acne. Why do we get acne scars? Scars are not only on the surface of the skin, but it is also beneath the surface of the skin. They eat away the collagen of the skin and leads to acne scars formation. Why do we get acne scars? There are many reasons. first, most common is acne. Because of acne, we get acne scars. People who have nodular acne, pus acne, reddish acne, they all lead to acne scar formation. Some people have the problem of getting scars from minor acne also. There are certain kind of medicines if you use for prolonged cases like steroid creams which make the skin thinner and lead to acne formation. Another important cause of acne is bursting and pricking acne. So, you should never do that. These are the causes of getting acne scars. What types of acne scars? There are many types of it. The most common thin one acne or prick scars. Other is rolling scars, box scars, hypertrophic scars, and keloidal scars. Multiple types of acne scars which a person can get. Now we come on the treatment of the acne scars.
What are the treatment options we have? We will start from the earlier modality which is most common now. So, the earliest one which was there is submission, punch grafting then derma roller, Co2 fractional laser, and the last treatment are MNRF. This is the latest one. So, in patients who are having scars which are multiple then we need to combine multiple treatments. If the patient has box scars with rolling scars then we combine the treatment. The patients who have ice pricks scars or box scars or rolling scars, we use MNRF in such cases. Now, we come to the side-effects of the treatment. First and the foremost thing that they do not go away with any cream. So, you have to do procedures for acne scars. What are the side-effects? There are chances of getting mild redness, swelling. These will remain for almost 24-48 hours. After this, your skin comes back to normal. The earlier treatment was a fractional laser, the downtime of the patient was close to around 5 days. So, that treatment has been replaced by MNRF and its downtime is only 36-48 hours. What is the post-operative instruction when you do some treatments for acne scars? Sun protection is very important because you have done some treatment on your face if you have exposure in the sun, then there are chances of getting pigmentation.
Post-procedure, you need to hydrate your skin. Drink a lot of water. Apply proper moisture. Anti-biotic creams will be recommended to you. Wash your face with a mild gentle cleanser. Do not rub, prick or scrub your face. These are the post-operative instruction we give to the patient. I would like to give some golden advice on acne scars. The first thing to treat acne scars is by getting treatment from a dermatologist at an early stage of acne. If you will take a longer time, then there are chances you might get multiple acne scars. I am Dr. Deepam Shah, Dermatologist. Today I will talk about PRP therapy. This is a very talked about the treatment which you might have heard on various platforms. It is very much in the limelight. It is not only used in dermatology but also in various fields like orthopedics, physiotherapy. So, I will discuss PRP in detail. Let's discuss the mechanism. How does it act? The entire treatment is based on platelets which are the white cells. They have a lot of growth factor-like endothelin, vascular endothelial, platelet drive growth factor and many. So, once we extract the platelets and give it to the area which is needed, it gives a boost to the area in which we are going to inject. What is the indication we look at in PRP therapy?
So, the first and the most common indication is hair. There are patients who suffer from androgenetic alopecia in a variety of grades, patients of hair thinning and hair fall, even in post-transplantation patients, we give the PRP treatment. 2nd, we come to the skin part. In the skin, it is used basically for cosmetic, anti-aging, anti-pigmentation, skin tightening because it boosts collagen regeneration, for skin lightening. Other then this it is being used for ulcerative conditions. So many patients have wounds and ulcer on the various parts of the body. So, in such cases, it is injected or applied topically and results in healing fastly. The procedure is very simple. We take out around 10-15 ml of the patient's blood and then we centrifuge it.
We separate the component from the blood. Then we divide the platelet into platelet-rich plasma. We increase the same by 4 times. After removing the platelets, we inject the same on the required area. If a patient is suffering any hair thinning or alopecia problem, we inject the same in the scalp area. If a patient has any skin problem, then we inject on the affected area of the face. How often do we repeat the PRP session? It can be repeated on an interval of 4 weeks. The minimum gap should be of 4 weeks and then we can do multiple sessions. In hair, generally, 4-5 sessions are recommended. For skin tightening and skin pigmentation, we generally recommend 3-4 sessions at an interval of a month.
But in ulcerative condition, we do it more often. At the interval of 1-2 weeks, PRP is recommended. We come to the final thing which is side-effects. Since it is a natural treatment because we take your own blood so it is highly sophisticated and without any side-effects because platelets are driven from your own body. The only side-effect can be the mild pain postoperatively which last for around 12-14 hours. So, this is the normal feeling when you inject something in your skin. So, this is about PRP. Side-effects are also less. It is divided into the 3 zones. Front zone, med scalp and the last part. The number of hair for transplantation depends on every individual. This procedure is highly sophisticated and should be done properly. This is a highly skilled operation. Look at the degree first of the Dr. and Dr experience. We need to take proper precaution while doing this procedure. The result of transplantation starts on an immediate basis. Hair starts to grow after 2 months of the procedure.
So, you can expect the best result within 10-12 months. The natural growth of the hair line is very important. No one comes to know whether you have gone through this procedure or not. The only side-effect is minor pain which can be handled by painkillers, postoperative swelling. When we see less hair on the head donor area then we have to take out the hair from some other area like chest, hand and legs. There is a myth that hair will not grow in transplantation. Fact is that it is a part of natural hair. So, it grows in a natural way. 2nd myth is that brain damage can also happen. Fact is that we use local anesthesia on the local area.
