Hello everyone, my name is Dr. Pallavi Singh. I am a physiotherapist for the last thirteen years and for nine years, I am practicing specifically in podiatry, podiatry is a science which is concerned with your foot alignment, gait and posture.
So what we typically see at our clinic is a lot of patient visit to us with chronic back pain. There are some people who have been like in sports or some activity and they had an injury which is causing the back pain, it is basically happening because of some immediate trauma. But, the chronic back pain is something that you are experiencing over a period of time, so you go to a doctor you get yourself treated sometimes it gets settled down with medication, sometimes you go to a physiotherapist you take five to ten session to rectify this pain and then it works, it works for three months, six months, but the pain keep on coming back again and now you get frustrated, you don't understand why this pain is happening to me? why am I getting that nagging pain on and on? and you are doing your level best in terms of exercises, modifications, precautions but still this pain is bothering. So maybe this time when you feel this kind of pain and this is not going back with your regular exercises and everything maybe you can consider getting your feet checked.
So what happens typically, your feet serve as the base of support for your whole body, now if you have a bad alignment at both the feet or sometimes at one feet so supposingly five or ten years back, you are walking around and you sprained your ankle so you can have bad alignment at one side or both sides. For one side, let's take an example you were playing, you sprain your ankle, your feet fall and twisted inside now sometimes this sprained get treated, sometimes we find like the injury was very less so we didn't go for a proper treatment or correction or anything of that sort and over a period of time these feet start getting more flattened and start turning inward. When this happens you have a unilateral collapse from one side, now as an effect of this your body is shortened on one side and lengthened on one side which is in terms you can say a leg length discrepancy, now this is not like a true shortening it's an apparent shortening.
When you are walking with this alignment the spine pays the price for it, so your pelvis which is supposed to be in square or centre started tilting towards one side, now this tilt can cause the rotation of your spine and can lead to further misalignment at the spinal level. This pain is something which is not treatable with normal exercises till the time you don't get your spine balanced and in the centre, this pain will keep on nagging and bothering you.
So a proper correction in this line would be getting the overpronation corrected on the right side, when the right side is levelled with the left side then the whole leg is in same alignment which will release the pressure from your spine and maybe this time your back pain will go for good. If you are having this chronic back pain again, now this time go for a foot evaluation maybe that can help you in solving the problem much faster.

I am Dr. Pallavi Singh. I am a physiotherapist for 14 years and I am practicing podiatry for the past 9 years. Podiatry is a field or branch which focuses on the lower limb. So what we do is, we assess people from various walks of life having issues with their foot. The most important section of it is the kids. The kids are our future so where our future is lying. Many a time we see that there are certain kids who are very good, very agile. They are running, jumping, doing well. But there are certain kids who are not being able to compete with them. They are falling, they are getting injuries, they are tripping. There are various reasons for this but one of the most common reason is flat foot.
A flat foot is not a physiological disease or a problem. It's just a structural issue. Certain kids after certain age start developing arches, certain kids does not develop the arches. Arches are important for having a proper bio-mechanics and it gives better shock absorption. So by the age of 5 years, kids should have a good arc. If it is still not developed and they are having issues with running and they are lazy, lacking behind, they don't want to get into physical activity then it can cause other health issues. Childhood obesity is already on the rise. So we as a responsible parent can take a call early in life and we can get the feet corrected. Kids from the age of 5 to 7 years are acceptable to all ortho therapy. Orthotics are nothing but special insert or a customized insole is inserted in the school shoes. So while you are buying the school shoes there are certain things you have to be cautious about.
Choose the shoes which are a proper size for them. Don't think that the kids are of growing age, so after 3 months or 6 months, we will have to change their shoes. Whenever you are buying the shoes it is as per the foot size. Only one finger gap should be there between the toe and the end of the shoes which is there for giving a little movement inside the shoes. If you buy one or two bigger size shoes then the archers are bound to get more flatten over the period of time rather than getting proper support. So the shoe size is important and necessary while you're buying it for your kids. Choose a shoe with proper size, with good stability for them. Even after that many time we see the kids are having difficulty in running, their knees go inside. So what we do is, we get these insole customized to them which help in creating a normal angle of the knee towards the foot and that helps in reducing the stress over the knee joint. Otherwise, the knee gets rubbed off and they can have issues when they go into adoloscent or later age of life. So for the better future, we have to act today.

I am Dr. Pallavi Singh, Podiatrist. Now in my day-to-day practice that most of the patients come to me with the morning heel pain. So, in the medical term, we call it as planterphasitis. Now, what is it? When you get up in the morning, you put your feet on the surface, you will have sharp, shooting pain. But after walking for a minute or so, the pain reduces. This keeps on happening regularly. Now slowly and steadily, it becomes a chronic pain and it is irritable. The way to confirm is the physical examination depending upon your history and lifestyle activity. We can do the x-ray to see the problem whether it is major or not. After identifying your problem, whether you can be treated conservatively.
In conservative treatment, we recommend physiotherapy. We suggest you a lot of exercises to strengthen your muscles. And we ask you to change your footwear. Footwear is the major cause of foot pain. So, always by footwear as per your foot type. The person has a 3 different kind of ailments. People can have a normal arch, low arch, and high arch. The footwear industry makes shoes for all kind of foot types. So, you need to identify, what is your foot type and buy a shoe accordingly. Now you have tried the conservative treatment, physiotherapy, stretching, changing the shoe but still, you are feeling the pain. What should you do? We scan your foot, we see the aliment and then we make a customized sole. We can take a pre-fabricated sole and we can add wedges in the sole to get your angels right. Another thing we do is heat the sole and mold as per your arches.
If there is any problem in aliment with the midfoot level, you will get overstretching of this facia which is causing you the pain. So, we stabilize your feet comfortably. Now, when the feet are completely supported by the soles, the pressure on the facia releases and the healing process accelerated. So, with all exercises and stretching, this works better. I hope you understood what I was trying to explain to you but I will still advise you to go and talk to your therapist, orthopedical, family physician and understand what is the best line of the treatment for you. Your Dr knows you better. Don't take a call on yourself. Because if you have this planterphasitis issue, it might need a customized sole. We are always there to help you.
