My dear friend in most of adult male some amount of balding/hair fall is natural & has nothing to do with masterbation. Hair transplant is standard treatment.

Tanning is effect of UV rays of sun & in my opinion glycolicacid (20%) peeling along with sun block gives best result.

It appears to be a case of vitiligo, if it is stable since last 2 years and no new patch appearing then melanocyte transfer surgery can help you.
