I have a keloid scar on my elbow from an accident over 5 years back. Over the time, it has increased and is itchy and painful on bumping into anything. What can be done to reduce the scar? Are there any kind of surgeries that could help flatten it?

Usually surgery is avoided. We give intralesional injections, which flattens the scar.

Pain in the scar suggests that it is still maturing. We would prefer to start Intralesional Triamcinolone along with compression, which should reduce the symptoms as well as the scar size. Surgery is reserved for resistant cases which do not respond better to the conservative regime.
