after taking Tripride HV 500/2/0.3 mg twice a day and insulin Mixtard 50/50 - 36 IU before breakfast and 24 IU before dinner and with my normal diet & exercise, my fasting sugar never comes below 220 and pp never below 340. Please improve/prescribe medicines.

lybrate-user, Thanks for the query. The glucose levels mentioned (Fasting 220 mg & PP 340 mg) are very high. These can lead to serious complications of diabetes like neuropathy, kidney damage, problems with retina, peripheral blood vessels leading to affection of blood supply to lower limbs. It is clear that present medication is not adequate at all. There has to be re-look at the total treatment, plus your diet and extent of daily exercise. Plus looking at your weight and height with a BMI of 31.14 kgs/M2 you are having grade 1 obesity. Which is a major contributor for improper glucose control. Your ideal body weight should be 63 kgs, hence there is an extra 27 kg load. Another aspect which needs to be closely looked at is what is your "normal diet" also exercise. Therefore, if you are keen enough, please come in a private conversation (paid consultation) with following details: current fasting, PP glucose levels, HbA1c%, lipid profile, extent of daily exercise, dietary pattern and BP readings.
