My wife had an ultrasound 3D/4D. She is 17 weeks pregnant. The doc while examining said everything is normal but in the report he wrote low lying placenta. Distance as 25 mm. Is there any risk? What precautions should be taken?

If you have low lying placenta you should restrict your activity, avoid strenous work ,prolonged standing and weight lifting. Maintain abstinence. As uterus grows low lying placenta migrates upwards, if it does not you may have caeserian section.
