If metastatectomy of the lung met is feasible then go ahead with it. It should be followed by Immunotherapy or targeted therapy if driver mutations are present. If Patient is not a candidate for metastatectomy, then directly consider for immunotherapy with pembrolizumab or nivolumab with or without ipilimumab, or targeted therapy if driver mutation is present. If progression on first line, then second line therapy may be considered. But the cost of immunotherapy is around 2 lakh rupees per month. Without any treatment we are looking at few months. With treatment we are looking at months to a couple of years.
