I am 23 (male). I have puffy nipples. I started gym 3 years back but still the problem persist. Due to which I could not go shirtless. I am not fat. I could see my abs but in chest area there is Fatty layer. Any suggestions to overcome this? Thank you.

It could be due to gynaecomastia or rarely due to benign lumps like lipoma or cancer. The gynaecomastia is usually due to the imbalance of estrogen and progesterone levels. It can have various causes like pubertal gynaecomastia, anabolic steroids used by body builders, drugs like anti psychotics, diuretics, steroids, omeprazole to name a few, testicular tumors, liver failure and hyperprolactinemia. Better to see a breast surgeon, who would examine you and if needed ask for an ultrasound and rule out the various etiologies. Feel free to contact me directly if you want to discuss this further.
