Hi, Need help with treatment doubts: My dad with Lung cancer has brain metastasis now with 1 T2 hypointense lesion in the left half of cerebellum of 23 mm, 31 mm, 18 mm. Primary lung tumor has reduced to a greater extent. He has difficulty in balancing. I want to know: - Is invasive Surgery the only option, can't it be treated with Stereotactic Radiosurgery? - How much time he has to go through the surgery as he is reluctant & not ready for it, we are trying to persuade him. - Can you suggest best hospital in Mumbai & costs for the surgery only. - Does the package of hospitals include everything or there are a lot of hidden charges when the package says 1.30 lc for the brain tumour surgery, Please help me to decide fast as we have very less time & our doctors are not that helpful.

I would be in a better position if you send me the details of the treatment given so far and the reports of radiology and pathology. For lung cancer (NSCLC) we prefer surgery for solitary lesion (it gives pathology which SRS cannot), or for lesions causing raised intracranial tension with risk of brain stem herniation. For those with multiple lesions without raised intracranial tension, if mutations are present, targeted therapy is first choice. For those without mutations, SRS is preferred. We do have stereotactic Radiosurgery at our institute, which is one of the best ones in Mumbai. If you can contact me directly with all the reports, I would be able to guide you better.
