Hello. My mom is 53 years old and she has kidney disease bilateral renal parenchyma her creatinine level is 4.3 and sodium level is 146.8 mmol/L but the main issue is that her kidneys are shrinked please guide us what we do?

If her kidney are shrinked and creatinine is around 4 better to start treatment by a nephrologist. Strictly follow his medical and dietary advice. With strict dietary requirements also sometimes the creatinine might continue to rise and she might require dialysis in future. So continue monitoring the creatinine and follow nephrologist advice.
