Premature ejaculation (pe) affects an estimated 30 percent of all men. While the condition is not fully understood in the medical community, it?s one that has been wildly debated in terms of effective treatment options. Men looking to cure or treat their pe are often willing to try anything. And, there are a handful of men out there who would prefer natural methods over pills or topical medication. Behavioral therapy is one treatment option that?s long been discussed. And for men hoping to not have to take a pill or buy a spray, gel or wipe, an option like behavioral therapy may seem like a good place to start. Behavioral therapies are treatment options, typically for mental health issues, that are based on the classical conditioning theory that behavior is learned, and can thus be changed through reinforcing more desirable behaviors. For years now, some experts have claimed that behavioral therapy can help treat pe. The two most common behavioral therapies for premature ejaculation are the ?squeeze technique? and the "start-stop technique. What is the squeeze technique? The squeeze technique is sometimes recommended as a therapy for pe. Just before ejaculation, the shaft of the penis is firmly squeezed between the thumb and forefinger. The squeezing causes a significant reduction in the erection, which ultimately prevents ejaculation. Many men claim the squeeze technique for premature ejaculation has helped them tremendously. However, most agree technique must be practiced in order to learn the proper timing, squeeze location and pressure. Some men have their partners participate and actually administer the squeeze technique. Several uncontrolled studies have indicated great success with the squeeze technique, but upon review of the literature, it appears that no formal clinical studies have been performed. Using the squeeze method for premature ejaculation is a two-step process. Once you?ve mastered the first step, you can just start on part two each subsequent time you use the technique. Step 1 ? learning the squeeze method: 1.You may want to ask for help from your partner to get fully erect. 2.Once your penis is erect, place your (or your partner?s) thumb on the frenulum. The frenulum is the string-like strip of skin that connects the penis head to the foreskin. 3.Next, put the pointer and middle finger on the back side of the penis, opposite where your thumb is. 4.Be sure the pointer and middle finger are on both sides of the coronal ridge. This is the slight ridge along the tip of the penis a couple centimeters from the top. Keep the fingers close to one another, but still on either side of the ridge. 5.Finally, squeeze the penis with both fingers and the thumb, holding the squeeze for approximately four seconds. You want to squeeze hard. If your partner is helping you, they probably won?t use enough pressure at first, so you might have to place your hands on top and squeeze with them until they get a feel for it. 6.Wait 15-30 seconds, and then begin to stimulate the penis again. You will likely lose some of your erection during the squeeze process, which is normal. 7.Once you are fully erect again, you can repeat the process around 4 or 5 times. 8.After a few sessions, you can move on to try and achieve ejaculation control. Step 2 ? using the squeeze method for ejaculatory control: 1.Masturbate until you?re almost ready to have an orgasm. 2.Use the squeeze technique like you learned in the above steps. 3.It?s very important to be sure you squeeze at the right time. If you feel like you may ejaculate immediately after you start again, you didn?t stop early enough. It?s common to have to try a few different sessions to get the timing right. 4.Repeat the method about 5 times, or for about 15-20 minutes, whichever comes first. 5.Be sure you have a pleasurable ejaculation and orgasm when you?re done. 6.Practice for 3-4 days. 7. Next, try the same technique during oral sex for another 3-4 days. 7.Finally, try the technique with full penetrating sex. 8.Once you?re to the point of actual intercourse, use the squeeze technique 2-3 before sex, during foreplay. What is the ?start-stop? method? The start-stop method is probably the most common behavioral therapy for premature ejaculation. The start-stop technique for pe involves learning to identify what?s often referred to as "mid-level excitement. The international society for sexual medicine's guidelines for the diagnosis and treatment of premature ejaculation claims that when a man learns to identify mid-level excitement, he can then prevent ejaculation. It?s worth noting, however, that beyond this claim, there isn?t much research to back up the theory that the start-stop technique works. To perform this method, you must stop stimulation (e.g. Intercourse) and allow the excitement level to diminish before beginning again. To use the start-stop method, men are generally instructed to begin with self-stimulation. When mid-level excitement is identified and ejaculation is controlled, you can then move on to partner hand stimulation, then to intercourse with minimal movement and finally to stop-start thrusting. Like the squeeze method, the start and stop method is a two-part process. Part 1: learning the start-stop method on your own 1.Frist, relax. Then begin to masturbate using only your hand. Don?t go too fast or hard ? slow and steady wins the race. 2.Once you feel almost ready to climax, stop. Timing is key ? you don?t want to stop too early, but also don?t want to go too far either. You may have to play around with the timing on this. That?s ok. 3.Once you stop, rest for 30 seconds to one minute. 4.Try to relax your entire body while taking slow, deep breaths. 5.Then, begin to masturbate again. 6.Repeat several times before finally allowing yourself to ejaculate and have an orgasm (don?t skip this last step ? it?s important to enjoy your orgasm). 7.After multiple sessions, you?ll may begin to notice longer periods of time between erection and ejaculation. 8.It can take 1-2 weeks for this method to work, so just keep trying. 9.Once you notice a difference, add lubricant. At first you may notice a slight decline, but don?t be discouraged?you should improve gradually. Part 2: using the start and stop technique with a partner 1.Start slow ? foreplay is key. Ask your partner to sensually caress and rub your body. 2.Next, ask your partner to masturbate you with only their hand. If this is all it takes for you to ejaculate, you may need to just do this step over multiple sessions to build up some endurance. 3.Follow the steps above where you get almost to the point of orgasm, but stop just before. Remember, timing may take some practice. 4.Once you feel in control during the masturbating session, move on to oral sex. 5.When you improve during oral sex, you?re ready to move on to penetration. This may take several sessions and weeks. Be patient. 6.When you?re using this method during penetrating sex, you can do a few different things during the ?stop? portion of the technique. Some men like to: stop completely; stop, pull out and change positions; or continue very slowly with shallow thrusts. 7.Take it slow. Build up to more adventurous sex.
