I take mirtazapine 15 mg at night. I?m myself a doctor (mbbs) who has been doing other jobs. I?m trying now to return to medicine. I?m looking for a duty doctor job in a hospital. One hospital told me I have to do 2 days working 8 am-4 pm then 2 days working 12 pm-8 pm and then 2 days working 8 pm-8 am. And 1 day off. My question is: how do I manage my mirtazapine medication since I need to sleep after taking it. Especially the changeover from 12-8 to 8-8 and then back to 8-4.

Dear Dr. lybrate-user, hope your depression has been lifted to go into responsible duty doctor's job. Mirtazapine's sedation will go away as you develop tolerance. So you can take daily at night. Usually you will get a day off when shift changes. Working continuously for 24 hours without sleep is not advisable for persons living with depression. So consider only one shift of 8 or 12 hours daily. You can request other hospitals to hire you for a single shift. Otherwise you need to manage with fluoxetine alone.

I think you should just tell the authorities that you are on this medication and cannot do a night shift.

I hope the medication has been prescribed by a psychiatrist and you do regular monitoring of the same. You can take mirtazapine daily just before your longest sleep. Good luck.
