Hi, poem is a relatively new procedure which involves cutting one layer of the muscular coat of the esophagus so as to relive the pressure at the junction between the food pipe and stomach when food passes down. Since this is a newer procedure, skills and results are variable. If you have not been relieved by the poems procedure, I would recommend you repeat a manometry and check the pressure at the ge junction. If the pressure is still high and doesn't reduce on swallowing, you will need a formal surgery, known as a cardiomyotomy, with possible extended myotomy of the esophagus. This can be done by keyhole surgery. However, you need a thorough evaluation before surgical intervention.
