Itching and pain with burning will be looking more like fissure in ano rather than piles I would suggest take laxatives syp looz 15 ml morning and night sitz bath (lukewarm water in tub with betadine 1 cup and sit over it immersing the anus for 15 mins morning and night) cont pilorute oint before passing stools and in night cont antibiotic hope this helps.
