Hello. I'm 32 weeks pregnant and on sertraline antidepressant - zosert 75 from 2 weeks. Still I am feeling quite depressed, crying spells, not interested in anything. Feeling very sad and hopeless. I'm not able to take care of my son or myself. And this guilt is eating me. What should I do now?

Kindly review with your psychiatrist and plan for treatment optimization with increasing dose of sertraline. This can be coupled or followed by augmentation with lamotrigine. Non pharmacological management would include relaxation techniques, psychotherapy. Also, bright light therapy should be considered as it is safe in pregnancy.

It is never recommended to continue any medications in pregnancy if not absolutely required, as it can harm the baby. However, if the situation demands it it can be prescribed. This requires the consent of the mother and father. Since you are already on zosert 75, you need to wait till another 2 weeks for review with your psychiatrist. A medication needs to be given a period of 4 weeks to assess its success. I suggest you visit your psychiatrist after 2 weeks and with your consent, he/she will definitely help you out.
