Hello doctor I am a kidney transplant recipient. Couple of months back I did my tacrolimus test. The doctor letter reduced my dose from 3 mg to 2.5 mg. (1.5 -1.0) it's time for me to do the tacrolimus test again. (pangraf) I wonder, at which trough should I give my blood. (morning or night, when should I go to give my blood) trough of 1.5 mg or 1 mg. Or is it irrelevant? Please advise.

Tacrolimus drug level testing is done just before the next dose ie trough that is usually in the morning hour before taking the tablet of 1.5. Testing should be repeated according to the timing post transplant. In the first year, repeated testing is to be done, in the later years, testing is done only after dose modification or any clinical event.
