My wife indira jamwal is patient of bipolar disorder .she is taking olimelt 2.5 and lopez md for this. Sometime she becomes aggressive and abusive and other time very soft and mild. Please advise.

That is exactly what bipolar is: wild swings from high to very low. This is a very debilitating disease and your wife has suffered with it for a long time. At her age, it makes it quite difficult, primarily because of her lack of motivation. I am assuming this to be so. I hope I am wrong. Any way what can be done, which I am sure has not been implemented so far, is that she needs to work with a counselor in tandem with the medication. The talk therapy will train her in certain skills and techniques which will stand her in good stead to cope with the daily challenges in life. Minus that she is only dependent on the medication which is essential to stabilize the chemical imbalance in her brain. Through the therapy, however, she will be in a relationship and will progress in her condition depending on her cooperation. With this combination and with a three-year episode-free progress she can be considered for weaning off of medication, very gradually and in a staggered manner. If the doctor is not aware of her difficulties, then inform him/her and review the medication or dosage.

The medication does not have a mood stabilizer, adding the same would benefit her a lot. Consult any psychiatrist. Also over a period of time it is wise to titrate ad stop lopez as it a benzodiazepine which is not advised on a longer run. Good luck.
