My spouse who is 4 months pregnant is suddenly having terrible pain on her left palm. The pain persists 24 hours. Due this she is unable to sleep at night. The pain is very severe and painful; along with this she also experiencing numbness and cramps. I am not sure if it’s a case of cts. Note: she has her daily dosage of folic, calcium and iron supplements on time.

I think by cts you mean carpal tunnel syndrome. As you described her condition and also from her past history of hypothyroidism and pcos, this points toward carpal tunnel syndrome in pregnancy, but for differential diagnosis you need to have a clinical examination and then only we can comment more.
