I am 34 years male having urinary issues like feeling urge to urinate, burning sensation pain in pelvis area/penis area some time. Urine routine and culture normal, uroflowmetry normal, usg kub normal, xray kub normal, kft normal problem since last 2.5 months. Consult almost 7 to 8 urologists, recently doctor prescribed semen culture and in semen culture stay growth of easily seen. Based on sensitivity doctor prescribed 5 days course of amikacin 500 mg injection and 2 weeks of bacstol 200. Injection course is completed and also 3 days of bacstol course done. Urine flow improved but pain in pelvis area, urge to urinate and burning sensation occasionally still there. Please advise.

Hi Mr. lybrate-user. What you are experiencing might be what we call chronic prostatitis/chronic pelvic pain syndrome (cpps). Though, you seem to have had urinary infection which might indicate towards chronic prostatitis. Cpps basically is a diagnosis of exclusion, when we find out nothing we bundle such patients into cpps. Chronic prostatitis seem to evolve from a long standing infection of prostate which is not severe enough at once, but keep on troubling the patient from time to time as infection aggravates. Add routine exercise like running, walking, yoga, cycling to your routine if you can. Medication wise, take tab alfuzosin 10 mg 1 tab hs (horra soni/before sleep to avoid dizzyness as it lowers blood pressure) and tab tadalafil 5 mg 1 tab once a day. Get a repeat urine culture done and a uroflowmetry too. Anticholinergics like mirabegron or tolterodine need not be taken if you do not have to visit loo often. Thanks.
